肥胖患者万古霉素相关急性肾损伤的临床特征及危险因素OA
Clinical characteristics and risk factors analysis of vancomycin-associated acute kidney injury in obese patients
目的 探讨肥胖患者使用万古霉素后发生急性肾损伤(AKI)的现状及其危险因素,并分析其临床转归,为合理用药提供依据.方法 收集2016年1月至2023年1月接受万古霉素治疗的肥胖患者(身体质量指数≥ 28.0 kg·mL-2)的人口学信息、住院情况、实验室检查数据、合并疾病及用药信息等资料.根据是否发生AKI,将患者分为AKI组和非AKI组,采用多因素Logistic回归法分析肥胖患者使用万古霉素后发生AKI的独立危险因素及肾功能恢复的影响因素.结果 共纳入516例使用万古霉素治疗的肥胖患者,其中男性占66.5%,年龄中位值为59岁,AKI患者77例(AKI发生率为14.9%).多因素Logistic回归显示,万古霉素用药疗程(OR=1.074,P=0.000)、高血压(OR=2.042,P=0.006)、合并使用血管加压药(OR=2.696,P=0.004)及入住 ICU(OR=1.794,P=0.026)是AKI的独立危险因素.进一步分析显示,入住ICU(OR=3.433,P=0.022)和使用造影剂(OR=5.908,P=0.034)可显著促进肾功能好转.结论 对于存在基础疾病、万古霉素用药疗程较长及入住ICU的肥胖患者,应加强万古霉素治疗药物监测,提高治疗安全性.
AIM To explore the current status and risk factors of vancomycin-associated acute kidney injury(AKI)in obese patients and analyze its clinical outcomes,providing evidence for rational drug use.METHODS A retrospective analysis was conducted on obese patients(BMI≥28.0 kg·m-2)treated with vancomycin at our hospital from January 2016 to January 2023.Demographic information,hospitalization details,laboratory data,comorbidities,and medication use were collected.Patients were divided into AKI and non-AKI groups based on the occurrence of vancomycin-associated AKI.Multivariate logistic regression was used to analyze the risk factors for vancomycin-associated AKI and related factors influencing renal function recovery.RESULTS A total of 516 patients were included,with males accounting for 66.5%and a median age of 59 years.AKI occurred in 77 patients,with an incidence rate of 14.9%.Multivariable logistic regression showed that vancomycin treatment duration(OR=1.074,P=0.000),hypertension(OR=2.042,P=0.006),concurrent use of vasopressors(OR=2.696,P=0.004),and ICU admission(OR=1.794,P=0.026)were independent risk factors for vancomycin-associated AKI.Further analysis revealed that ICU admission(OR=3.433,P=0.022)and contrast agent use(OR=5.908,P=0.034)significantly promoted renal function improvement.CONCLUSION For obese patients with underlying diseases,prolonged vancomycin treatment or ICU admission,enhanced therapeutic drug monitoring is necessary to optimize treatment safety.
