基于超声多模态成像技术评估慢性肾病分期的临床研究OA
Clinical Research on the Assessment of Chronic Kidney Disease Staging Based on Ultrasound Multimodal Imaging Technology
目的 探讨超声多模态成像技术在慢性肾病(CKD)临床分期评估中的应用价值.方法 选取 2023年 1 月至 2025 年12 月南京市高淳人民医院收治的 90 例 2 型糖尿病肾病所致 CKD 患者(G1~G5 期)作为研究组,另选取 30 例健康体检者作为对照组.所有研究对象均行常规二维及彩色多普勒超声、实时剪切波弹性成像(SWE)、超声造影(CEUS)检查,检测相关超声参数,探讨各参数与 CKD 分期的相关性及评估效能.结果 随着 CKD分期进展,各组的肾主动脉、段动脉、小叶间动脉峰值流速(PSV)逐渐降低,阻力指数(RI)逐渐升高;肾实质杨氏模量值、CEUS 达峰时间(TTP)逐渐升高,CEUS 上升支斜率(A)、峰值强度(PI)逐渐降低,组间比较差异均有统计学意义(P<0.05).相关性分析显示,肾实质杨氏模量值、小叶间动脉RI、PI 与 CKD 分期相关性最高(r 绝对值均>0.7).ROC 曲线分析显示,三者联合检测评估 CKD 分期的曲线下面积(AUC=0.918)高于单一参数,灵敏度和特异度分别达 94.17%、93.33%.结论 超声多模态成像技术可反映 CKD 患者的肾实质结构、血流灌注及纤维化程度,各参数与CKD分期密切相关,联合检测可显著提高CKD分期评估效能.
Objective To explore the application value of ultrasound multimodal imaging technology in the clinical staging assessment of chronic kidney disease(CKD).Methods A total of 90 patients with type 2 diabetic nephropathy-induced CKD(staged from G1 to G5)admitted to Nanjing Gaochun People's Hospital from January 2023 to December 2025 were selected as the study group,and another 30 healthy subjects were selected as the control group.All subjects underwent routine two-dimensional and color Doppler ultrasound,real-time shear wave elastography(SWE),and contrast-enhanced ultrasound(CEUS)examinations to measure relevant ultrasound parameters and explore the correlations and evaluation efficacy of each parameter with CKD staging.Results As CKD staging progressed,the peak systolic velocity(PSV)of the renal aorta,segmental artery,and interlobular artery gradually decreased,while the resistance index(RI)gradually increased;the Young's modulus value of the renal parenchyma and the time to peak(TTP)of CEUS gradually increased,while the ascending slope(A)and peak intensity(PI)of CEUS gradually decreased,with statistically significant differences between groups(P<0.05).Correlation analysis showed that the Young's modulus value of the renal parenchyma,RI of the interlobular artery,and PI had the highest correlation with CKD staging(allrabsolute values>0.7).Receiver operating characteristic(ROC)curve analysis revealed that the area under the curve(AUC=0.918)for the combined detection of these three parameters in evaluating CKD staging was higher than that for a single parameter,with the sensitivity and specificity reaching 94.17%and 93.33%,respectively.Conclusions Ultrasound multimodal imaging technology can reflect the renal parenchyma structure,blood perfusion,and fibrosis degree in CKD patients,and each parameter is closely related to CKD staging.Combined detection can significantly improve the evaluation efficacy of CKD staging.
