头颈部超声联合CTA对颈动脉和颅内动脉狭窄的诊断价值OA
Diagnostic Value of Head and Neck Ultrasound Combined with CTA for Carotid and Intracranial Artery Stenosis
目的 探讨头颈部超声联合 CT血管造影(CTA)对颈动脉和颅内动脉狭窄的诊断价值.方法 回顾性分析 2023年 8月至2024年4 月新乡市第二人民医院收治的 90例颈动脉和颅内动脉狭窄患者的临床资料,所有患者均在入院 72 h 内依次完成头颈部超声、CTA 及数字减影血管造影(DSA)检查.以 DSA 检查结果为金标准,展开一致性分析,并评估头颈部超声、CTA 以及二者联合检查对颈动脉和颅内动脉狭窄的诊断价值.结果 DSA 检查结果显示,90例患者中颈动脉和颅内动脉轻度狭窄 39 例、中度狭窄33例、重度狭窄/闭塞18例.以 DSA检查结果为金标准,头颈部超声诊断轻度、中度、重度狭窄及闭塞的 Kappa 值分别为0.539、0.370、0.621,CTA的Kappa值分别为 0.768、0.590、0.722,头颈部超声联合 CTA的 Kappa 值分别为 0.909、0.836、0.894,联合检查在各级狭窄诊断中均表现出最优的一致性.相较于单一检查,头颈部超声联合 CTA 检查对轻度、中度狭窄的灵敏度、特异度、准确度更高,诊断效能更优(P<0.05).结论 头颈部超声与 CTA 对颈动脉和颅内动脉狭窄均有一定诊断价值,二者联合能够优势互补,进一步提高诊断准确性与可靠性.
Objective To investigate the diagnostic value of head and neck ultrasound combined with CT angiography(CTA)for carotid and intracranial artery stenosis.Methods The clinical data of 90 patients with carotid and intracranial artery stenosis admitted to Xinxiang Second People's Hospital from August 2023 to April 2024 were retrospectively analyzed.All patients underwent head and neck ultrasound,CTA,and digital subtraction angiography(DSA)examinations sequentially within 72 hours of admission.Taking DSA results as the gold standard,the consistency analysis was performed,and the diagnostic value of head and neck ultrasound,CTA,and their combination for carotid and intracranial artery stenosis was evaluated.Results DSA examination results showed that among the 90 patients,there were 39 cases of mild stenosis,33 cases of moderate stenosis,and 18 cases of severe stenosis/occlusion of the carotid and intracranial arteries.Taking DSA results as the gold standard,the Kappa values of head and neck ultrasound for mild,moderate,and severe stenosis/occlusion were 0.539,0.370,and 0.621,respectively;those of CTA were 0.768,0.590,and 0.722,respectively;and those of head and neck ultrasound combined with CTA were 0.909,0.836,and 0.894,respectively.The combined examination demonstrated the optimal diagnostic consistency for all grades of stenosis.Compared with single examination,head and neck ultrasound combined with CTA showed higher sensitivity,specificity and accuracy for mild and moderate stenosis,with better diagnostic efficacy(P<0.05).Conclusions Head and neck ultrasound,as well as CTA have certain diagnostic value for carotid and intracranial arterial stenosis.Their combination achieves complementary advantages and further improves diagnostic accuracy and reliability.
