目的 探讨接触式成像术中可视技术在高血压脑出血(HICH)手术治疗中的应用效果.方法 回顾性分析 2021年 1月至 2023年 8 月宜春市人民医院神经外科收治的100例 HICH 患者的临床资料,按照手术方式的不同分为对照组(69 例)及观察组(31例).对照组接受开颅血肿清除术治疗,观察组接受接触式成像术中可视技术辅助开颅血肿清除术治疗.比较两组的手术时间、术中出血量、术后残余血肿量、脑血肿清除率;对两组患者随访 12 个月,采用格拉斯哥预后评分(GOS)评估患者预后情况,并统计两组患者的术后再出血发生率.结果 观察组的手术时间短于对照组,术中出血量、术后残余血肿量均少于对照组(P<0.05).观察组的脑血肿近全清除率高于对照组,GOS分级优于对照组(P<0.05).两组的血肿基本清除率、部分清除率、术后再出血发生率比较,差异无统计学意义(P>0.05).结论 接触式成像术中可视技术应用于 HICH 手术中,可缩短手术时间,减少术中出血量及术后残余血肿量,提高脑血肿近全清除率,改善患者远期预后,具有显著的临床应用价值.

Objective To investigate the application effect of contact imaging-based intraoperative visualization technology in the surgical treatment of hypertensive intracerebral hemorrhage(HICH).Methods A retrospective analysis was performed on the clinical data of 100 patients with HICH admitted to the Department of Neurosurgery of Yichun People's Hospital from January 2021 to August 2023.According to different surgical methods,the patients were divided into the control group(n=69)and the observation group(n=31).The control group received craniotomy for hematoma evacuation,while the observation group received craniotomy for hematoma evacuation assisted by contact imaging-based intraoperative visualization technology.The operation time,intraoperative blood loss,postoperative residual hematoma volume,and cerebral hematoma clearance rate were compared between the two groups.The patients were followed up for 12 months,and the Glasgow Outcome Scale(GOS)was used to evaluate the prognosis of the patients.The incidence of postoperative rebleeding was also statistically analyzed.Results The operation time of the observation group was shorter than that of the control group,and the intraoperative blood loss and postoperative residual hematoma volume were less than those of the control group(P<0.05).The near-total cerebral hematoma clearance rate of the observation group was higher than that of the control group,and the GOS grade was better than that of the control group(P<0.05).There were no statistically significant differences in the basic hematoma clearance rate,partial hematoma clearance rate,and incidence of postoperative rebleeding between the two groups(P>0.05).Conclusions The application of contact imaging-based intraoperative visualization technology in HICH surgery can shorten the operation time,reduce intraoperative blood loss and postoperative residual hematoma volume,improve the near-total cerebral hematoma clearance rate,and improve the long-term prognosis of patients,which has significant clinical application value.
