温阳通络方联合扶阳罐治疗糖尿病周围神经病变阳虚寒凝证临床研究OA

Objective:To observe the clinical effect of Wenyang Tongluo Prescription combined with Fuyang cupping on diabetic peripheral neuropathy(DPN)with syndrome of cold accumulation due to yang deficiency.Methods:A total of 108 DPN patients admitted to Wenling Hospital of Traditional Chinese Medicine from May 2023 to March 2024 were selected and divided into the study group and the control group according to the random number table method,with 54 cases in each group.One case was lost to follow-up in the study group,and one case withdrew from the study due to personal reasons.Finally,52 cases were included.In the control group,one case was excluded due to the failure to complete the prescribed course of treatment according to the research plan,and one case was excluded due to the use of drugs that affected the results of the study.Finally,52 cases were included.Both groups were given Metformin alone or in combination with insulin for hypoglycemic treatment.The control group was given Mecobalamin Tablets and Epalrestat Tablets orally.The study group was additionally given Wenyang Tongluo Prescription combined with Fuyang cupping based on the treatment in the control group.Both groups were treated for eight weeks.The clinical efficacy of the two groups was evaluated,and the traditional Chinese medicine syndrome scores,Toronto Clinical Scoring System(TCSS)scores,pain scores,nerve conduction velocity,blood glucose,hemodynamics,and safety were compared between the two groups.Results:The total effective rate was 94.23%(49/52)in the study group,higher than that of 80.77%(42/52)in the control group(P＜0.05).After treatment,the TCSS scores,Visual Analogue Scale(VAS)scores,and traditional Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment(P＜0.05);the above scores in the study group were lower than those in the control group(P＜0.05).After treatment,the motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of common peroneal nerve and median nerve in the two groups were increased when compared with those before treatment(P＜0.05);the MNCV and SNCV of the common peroneal nerve and median nerve in the study group were faster than those in the control group(P＜0.05).After treatment,the levels of glycated hemoglobin A1c(HbA1c),fasting blood glucose(FBG),and 2-hour postprandial glucose(2hPG)in the two groups were decreased when compared with those before treatment(P＜0.05).The levels of HbA1c,FBG,and 2hPG in the study group were lower than those in the control group(P＜0.05).After treatment,the blood flow and peak systolic velocity(PSV)of dorsalis pedis artery and Ankle Brachial Index(ABI)in the two groups were increased when compared with those before treatment(P＜0.05);the blood flow and PSV of dorsalis pedis artery and ABI in the study group were higher than those in the control group(P＜0.05).There were no obvious adverse reactions in the two groups during the treatment.Conclusion:The therapy of Wenyang Tongluo Prescription combined with Fuyang cupping has a good effect on DPN with syndrome of cold accumulation due to yang deficiency,which can further reduce clinical symptoms,reduce blood glucose,accelerate nerve conduction velocity,and regulate hemodynamics with good safety.
