苍附导痰汤加减治疗多囊卵巢综合征合并胰岛素抵抗临床研究OA
Clinical Study on Modified Cangfu Daotan Decoction for Polycystic Ovary Syndrome Complicated by Insulin Resistance
目的:观察苍附导痰汤加减治疗痰瘀互结型多囊卵巢综合征(PCOS)合并胰岛素抵抗(IR)的临床疗效以及对患者肠道菌群丰度的影响.方法:回顾性分析2021年11月—2022年11月于广州中医药大学第一附属医院妇科门诊诊断为痰瘀互结型PCOS合并IR的30例患者的临床资料,设为PCOS-IR组,予苍附导痰汤加减治疗,以1个月经周期为1个治疗周期,治疗3个周期.治疗前后评估中医证候评分、月经周期评分,测量体质量、腰围、臀围,计算体质量指数与腰臀比,检测性激素指标[睾酮(T)、促卵泡激素(FSH)、促黄体生成激素(LH)、LH/FSH比值]与糖代谢指标[空腹血糖(FBG)、空腹胰岛素(FINS)、IR指数(HOMA-IR)].并选取同期于广州中医药大学第一附属医院体检的30例健康女性设为健康对照组,采用16S rDNA测序技术分析PCOS-IR组治疗前、治疗3个月经周期后及健康对照组的肠道微生物组成,以及门、纲、目、科、属水平上各菌种的相对丰度.结果:PCOS-IR组经过治疗后,总有效率为93.33%(28/30);中医证候积分、月经周期积分、LH、T、FBG、FINS、HOMA-IR值及LH/FSH比值均较治疗前下降,差异均有统计学意义(P<0.05);体质量指数、腰臀比及FSH水平治疗前后比较,差异均无统计学意义(P>0.05).健康对照组、PCOS-IR组治疗前与PCOS-IR组治疗后共同拥有的核心肠道菌群数量为443个.在门水平上,PCOS-IR组治疗前的厚壁菌门、拟杆菌门、变形菌门及疣微菌门相对丰度均高于健康对照组,放线菌门的相对丰度低于健康对照组,差异均有统计学意义(P<0.05).PCOS-IR组治疗后的放线菌门相对丰度较治疗前升高,厚壁菌门、拟杆菌门及变形菌门的相对丰度均较治疗前降低,差异均有统计学意义(P<0.05).在纲水平上,PCOS-IR组治疗前的梭状芽孢杆菌纲、拟杆菌纲及厚壁菌纲相对丰度均高于健康对照组,放线菌纲的相对丰度低于健康对照组,差异均有统计学意义(P<0.05).PCOS-IR组治疗后的拟杆菌纲、放线菌纲相对丰度均较治疗前升高,梭状芽孢杆菌纲、厚壁菌纲及变形菌纲的相对丰度均较治疗前降低,差异均有统计学意义(P<0.05).在目水平上,PCOS-IR组治疗前的梭菌目、双歧杆菌目相对丰度均低于健康对照组,拟杆菌目、肠杆菌目及月形单孢菌目的相对丰度均高于健康对照组,差异均有统计学意义(P<0.05).PCOS-IR组治疗后的梭菌目、双歧杆菌目相对丰度均较治疗前升高,拟杆菌目、月形单孢菌目的相对丰度均较治疗前降低,差异均有统计学意义(P<0.05).在科水平上,PCOS-IR组治疗前的瘤胃球菌科、双歧杆菌科相对丰度均低于健康对照组,普雷沃氏菌科、拟杆菌科的相对丰度均高于健康对照组,差异均有统计学意义(P<0.05).PCOS-IR组治疗后的瘤胃球菌科、双歧杆菌科相对丰度均较治疗前升高,普雷沃氏菌科、拟杆菌科的相对丰度均较治疗前降低,差异均有统计学意义(P<0.05).在属水平上,PCOS-IR组治疗前的双歧杆菌属、粪杆菌属及瘤胃球菌属相对丰度均低于健康对照组,普雷沃氏菌属、埃希氏菌-志贺氏菌属的相对丰度均高于健康对照组,差异均有统计学意义(P<0.05).PCOS-IR组治疗后的双歧杆菌属、瘤胃球菌属相对丰度均较治疗前升高,埃希氏菌-志贺氏菌属、粪杆菌属的相对丰度均较治疗前降低,差异均有统计学意义(P<0.05).PCOS-IR组治疗前的shannon指数低于健康对照组(P<0.05).结论:采用苍附导痰汤加减治疗痰瘀互结型PCOS合并IR,可有效缓解痰瘀互结证相关中医证候,调节性激素水平,改善排卵和IR情况,进而调节月经节律,其可能通过正向调整肠道微生物群落结构,如增加双歧杆菌等有益菌的丰度,同时调节厚壁菌门、拟杆菌门和埃希氏菌-志贺氏菌属等与PCOS-IR相关的菌群组成,改善肠道微生态平衡而产生疗效.
Objective:To observe the clinical efficacy of modified Cangfu Daotan Decoction on patients with polycystic ovary syndrome(PCOS)of phlegm blended with blood stasis type complicated by insulin resistance(IR),and to explore its effect on intestinal flora abundance.Methods:Retrospective analysis was given to the clinical data of 30 patients diagnosed with PCOS of phlegm blended with blood stasis type complicated by IR in the gynecological clinic of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from November 2021 to November 2022.They were assigned to the PCOS-IR group and treated with modified Cangfu Daotan Decoction for three courses of treatment,with one menstrual cycle as one course.Before and after treatment,the traditional Chinese medicine syndrome scores and menstrual cycle scores were evaluated,the body mass,waist circumference,and hip circumference were measured,the body mass index and waist-to-hip ratio were calculated,and the sex hormone indexes[testosterone(T),follicle stimulating hormone(FSH),luteinizing hormone(LH),LH/FSH ratio]and glucose metabolism indexes[fasting plasma glucose(FBG),fasting insulin(FINS),Homeostasis Model Assessment of Insulin Resistance(HOMA-IR)]were detected.A total of 30 healthy women who underwent physical examination in the First Affiliated Hospital of Guangzhou University of Chinese Medicine at the same period were selected as the healthy control group.The 16S rDNA sequencing technology was used to analyze the intestinal microbial composition of the PCOS-IR group before treatment,after three menstrual cycles of treatment and of the healthy control group,as well as the relative abundance of each strain at the level of phylum,class,order,family and genus.Results:After treatment,the total effective rate was 93.33%(28/30)in the PCOS-IR group;the traditional Chinese medicine syndrome scores,menstrual cycle scores,LH,T,FBG,FINS,HOMA-IR,and LH/FSH ratio decreased compared with those before treatment,differences being significant(P<0.05);there were no significant differences being found in the comparisons of body mass index,waist-to-hip ratio,and FSH level before and after treatment(P>0.05).A total of 443 core gut microbiota are shared among the healthy control group,the PCOS-IR group before treatment,and the PCOS-IR group after treatment.At the phylum level,the relative abundance of Firmicutes,Bacteroidetes,Proteobacteria,and Verrucomicrobia in the PCOS-IR group before treatment was higher than that in the healthy control group,and the relative abundance of Actinobacteria was lower than that in the healthy control group,differences being significant(P<0.05).The relative abundance of Actinobacteria in the PCOS-IR group after treatment was increased when compared with that before treatment,and the relative abundance of Firmicutes,Bacteroidetes,and Proteobacteria decreased compared with that before treatment,differences being significant(P<0.05).At the class level,the relative abundance of Clostridia,Bacteroidia,and Negativicutes in the PCOS-IR group before treatment was higher than that in the healthy control group,and the relative abundance of Actinobacteria was lower than that in the healthy control group,differences being significant(P<0.05).The relative abundance of Bacteroidia and Actinobacyeria in the PCOS-IR group after treatment increased compared with that before treatment,and the relative abundance of Clostridia,Negativicutes,and Gammaproteobacteria decreased compared with that before treatment,differences being significant(P<0.05).At the order level,the relative abundance of Clostridiales and Bifidobacteriales in the PCOS-IR group before treatment was lower than that in the healthy control group,and the relative abundance of Bacteroidales,Enterobacteriales,and Selenomonadales was higher than that in the healthy control group,differences being significant(P<0.05).The relative abundance of Clostridiales and Bifidobacteriales in the PCOS-IR group after treatment elevated compared with that before treatment,and the relative abundance of Bacteroidales and Selenomonadales reduced compared with that before treatment,differences being significant(P<0.05).At the family level,the relative abundance of Ruminococcaceae and Bifidobacteriaceae in the PCOS-IR group before treatment was lower than that in the healthy control group,and the relative abundance of Prevotellaceae and Bacteroidaceae was higher than that in the healthy control group,differences being significant(P<0.05).The relative abundance of Ruminococcaceae and Bifidobacteriaceae in the PCOS-IR group after treatment was up-regulated compared with that before treatment,and the relative abundance of Prevotellaceae and Bacteroidaceae was down-regulated compared with that before treatment,differences being significant(P<0.05).At the genus level,the relative abundance of Bifidobacterium,Faecalibacterium,and Ruminococcus in the PCOS-IR group before treatment was lower than that in the healthy control group,and the relative abundance of Prevotella and Escherichia-Shigella was higher than that in the healthy control group,differences being significant(P<0.05).The relative abundance of Bifidobacterium and Ruminococcus in the PCOS-IR group after treatment elevated compared with that before treatment,and the relative abundance of Escherichia-Shigella and Faecallibacterium reduced compared with that before treatment,differences being significant(P<0.05).The Shannon index in the PCOS-IR group before treatment was lower than that in the healthy control group(P<0.05).Conclusion:The application of modified Cangfu Daotan Decoction in the treatment of PCOS of phlegm blended with blood stasis type complicated by IR can effectively alleviate the related syndromes,regulate the level of sex hormones,improve ovulation and IR,and then regulate the menstrual rhythm.It may positively regulate the intestinal microbial community structure,such as increasing the abundance of beneficial bacteria such as Bifidobacterium,and regulating the composition of flora related to PCOS-IR such as Firmicutes,Bacteroidetes,and Escherichia-Shigella,thereby improving the intestinal microecological balance.
