目的 基于转化生长因子-β1(TGF-β1)/Notch1/p53 信号通路探讨加味五子衍宗丸减轻高尿酸血症诱导大鼠足细胞损伤的机制.方法 将60 只 SPF 级 SD 雄性大鼠随机分为正常组、模型组、别嘌醇组、加味五子衍宗丸低剂量组、加味五子衍宗丸中剂量组、加味五子衍宗丸高剂量组,每组10 只.除正常组外,其他组大鼠建立高尿酸血症模型,然后加味五子衍宗丸低、中、高剂量组分别给予0.72 g/(kg·d)、1.44 g/(kg·d)、2.88 g/(kg·d)加味五子衍宗丸灌胃,别嘌醇组给予25 mg/(kg·d)别嘌醇灌胃,模型组和正常组给予等体积的生理盐水灌胃,均 1 次/d,连续灌胃8 周.Biacore 实验验证加味五子衍宗丸与 TGF-β1 蛋白结合情况,按试剂盒步骤检测血尿酸(SUA)、血肌酐(SCr)、尿素氮(BUN)水平及尿微量白蛋白/尿肌酐比值(ACR),HE 染色观察肾组织病理形态,免疫组化法观察肾组织中 TGF-β1、Notch1、p53、Nephrin 蛋白阳性表达情况,qRT-PCR 法检测肾组织中 TGF-β1、Notch1、p53、Nephrin mRNA 表达情况.结果 加味五子衍宗丸与TGF-β1 存在分子间的结合作用.与正常组比较,模型组血清 SUA、SCr、BUN 水平及 ACR 均明显升高(P 均<0.05);肾小球萎缩,肾小管近曲小管、远曲小管水肿,可见炎性细胞浸润并伴有细胞空泡化;肾组织中 TGF-β1、Notch1、p53 蛋白阳性表达评分及 mRNA 相对表达量均明显升高(P 均<0.05),Nephrin 蛋白阳性表达评分及 mRNA 相对表达量均明显降低(P 均<0.05).与模型组比较,各药物组血清 SUA、SCr、BUN(除加味五子衍宗丸低剂量组)水平及 ACR 均明显降低(P 均<0.05);肾组织病理变化有不同程度缓解;肾组织中 TGF-β1、Notch1、p53 蛋白阳性表达评分均明显降低(P 均<0.05),别嘌醇组和加味五子衍宗丸高剂量组 Nephrin 蛋白阳性表达评分均明显升高(P 均<0.05);各药物组 TGF-β1、Notch1、p53 mRNA 相对表达量均明显降低(P 均<0.05),Nephrin mRNA 相对表达量均明显升高(P 均<0.05).各药物组间比较,加味五子衍宗丸高剂量组各指标改善更明显.结论 加味五子衍宗丸可通过调控 TGF-β1/Notch1/p53 信号通路来减轻高尿酸血症诱导的大鼠足细胞损伤.

Objective It is to explore the mechanism of modified Wuzi Yanzong Pill in attenuating podocyte injury in-duced by hyperuricemia in rats via the transforming growth factor-β1(TGF-β1)/Notch1/p53 signaling pathway.Methods Sixty SPF-grade male SD rats were randomly divided into normal group,model control group,allopurinol group,and low-dose,medium-dose,and high-dose groups of modified Wuzi Yanzong Pill,with 10 rats in each group.The rats of all groups except for the normal group were prepared to establish models of hyperuricemia.The low-dose,medium-dose,and high-dose groups of modified Wuzi Yanzong Pill were given modified Wuzi Yanzong Pill0.72 g/(kg·d),1.44 g/(kg·d),2.88 g/(kg·d)via gavage,respectively,the allopurinol group was given allopurinol 25 mg/(kg·d)via gavage,the model group and normal group were given equal volume of normal saline via gavage,all once daily,continuously treated for 8 weeks.The binding of modified Wuzi Yanzong Pill with TGF-β1 protein was verified by Biocore experiment,the levels of serum uric acid(SUA),serum creatinine(SCr),blood urea nitrogen(BUN),and albumin-to-creatinine ratio(ACR)were measured according to the kit instructions,the morphology of renal tissue was observed by HE staining,the protein ex-pressions of TGF-β1,Notch1,p53,and Nephrin in renal tissue were detected by immunohistochemistry,the mRNA ex-pressions of TGF-β1,Notch1,p53,and Nephrin in renal tissue were detected by qRT-PCR.Results Modified Wuzi Yan-zong Pill had a molecular binding with TGF-β1.Compared with the normal group,the levels of SUA,SCr,BUN and ACR were all significantly increased in the model group(all P<0.05);glomerular atrophy,edema of proximal and distal con-voluted tubules,as well as inflammatory cell infiltration accompanied by cellular vacuolization were found in the model group;the positive expression scores and relative mRNA expressions of TGF-β1,Notch1,and p53 in renal tissue were all significantly increased(all P<0.05),while the positive expression score and relative mRNA expression of Nephrin in re-nal tissue were both significantly reduced(all P<0.05).Compared with the model group,the levels of SUA,SCr,BUN(except for the low-dose group of modified Wuzi Yanzong Pill)and ACR were all significantly decreased in the medicated groups(all P<0.05);the morphologic changes in renal tissue were attenuated at different degrees in the medicated groups;the positive expression scores in renal tissue were all significantly decreased(all P<0.05),while the positive ex-pression scores of Nephrin in renal tissue of the allopurinol group and high-dose group of modified Wuzi Yanzong Pill were all significantly increased(all P<0.05);the relative mRNA expressions of TGF-β1,Notch1,and p53 in renal tissue of the medicated groups(all P<0.05),while the relative mRNA expressions of Nephrin in renal tissue of all medicated groups were all significantly increased(all P<0.05).The improvements of these indicators were the most significant in the high-dose group of modified Wuzi Yanzong Pill when compared between the medicated groups.Conclusion Modified Wuzi Yanzong Pill can attenuate podocyte injury induced by hyperuricemia in rats via regulating the TGF-β1/Notch1/P53 signaling pathway.
