循证导向的吞咽康复分层策略联合动态营养管理方案在老年脑卒中患者中的应用研究OA
Application of evidence-based stratified swallowing rehabilitation strategy combined with dynamic nutrition management in elderly stroke patients
目的 探讨循证导向的吞咽康复分层策略联合动态营养管理对老年脑卒中患者吞咽功能及营养状态的影响.方法 选取2024年1月至2025年2月该院198例老年脑卒中患者,采用随机信封法将其分为对照组(99例)和观察组(99例).对照组采用常规康复干预联合动态营养管理方案,观察组在对照组基础上联合循证导向的吞咽康复分层策略.比较2组 Gugging吞咽功能评估量表(GUSS)、吞咽功能筛查量表(EAT-10)、微型营养评定简表(MNA-SF)评分,以及血清前白蛋白(PA)水平、握力与不良事件发生率.结果 干预后,2组GUSS、EAT-10评分优于干预前,且观察组各指标优于对照组,差异有统计学意义(P<0.05).干预后,2组 MNA-SF评分、血清PA水平及握力高于干预前,且观察组各指标高于对照组,差异有统计学意义(P<0.05).观察组误吸及其他不良事件总发生率为5.05%,低于对照组的13.13%,差异有统计学意义(P<0.05).结论 循证导向的吞咽康复分层策略联合动态营养管理能改善老年脑卒中患者吞咽功能及营养状态,减少误吸等不良事件发生风险.
Objective To investigate the effects of evidence-based stratified swallowing rehabilitation strategy combined with dynamic nutrition management on swallowing function and nutritional status in elder-ly stroke patients.Methods A total of 198 elderly stroke patients in our hospital from January 2024 to Febru-ary 2025 were selected and randomly divided into a control group(99 cases)and an observation group(99 ca-ses)using a random envelope method.The control group received conventional rehabilitation intervention combined with dynamic nutrition management,while the observation group received evidence-based stratified swallowing rehabilitation strategy in addition to the intervention given to the control group.The Gugging Swallowing Screen(GUSS),Eating Assessment Tool-10(EAT-10),Mini-Nutritional Assessment Short-Form(MNA-SF)scores,serum prealbumin(PA)level,grip strength,and incidence of adverse events were compared between the two groups.Results After intervention,the GUSS and EAT-10 scores in both groups improved compared with those before intervention,and the observation group showed better Results than the control group,with statistically significant differences(P<0.05).After intervention,the MNA-SF scores,serum PA level,and grip strength in both groups were higher than those before intervention,and the observation group had higher values than the control group,with statistically significant differences(P<0.05).Regarding the in-cidence of aspiration and other adverse events,the total incidence in the observation group was 5.05%,which was lower than that in the control group(13.13%),with a statistically significant difference(P<0.05).Con-clusion Evidence-based stratified swallowing rehabilitation strategy combined with dynamic nutrition man-agement can improve swallowing function and nutritional status in elderly stroke patients and reduce the risk of adverse events such as aspiration.
