目的 探讨扫描式葡萄糖监测(FGM)系统对成人糖尿病患者血糖控制及知-信-行(KAP)水平的影响.方法 选取2022年1月至2024年1月该院230例糖尿病患者,按随机数字表法将其分为观察组和对照组,每组115例.对照组采用指尖血糖检测与胰岛素泵治疗方案,观察组采用FGM 联合胰岛素泵治疗方案.比较2组血糖生化指标[空腹血糖(FPG)、餐后2小时血糖(2hPG)、糖化血红蛋白(HbA1c)]、血糖波动性指标[平均血糖波动幅度(MAGE)、血糖标准偏差(SDBG)、血糖日间平均差异(MODD)],以及血糖达标时间、胰岛素用量、低血糖人均次数、血糖达标率、KAP评分.结果 2组干预前 HbA1c、FPG、2hPG水平比较,差异无统计学意义(P>0.05).干预后,2组 HbA1c、FPG、2hPG水平低于干预前,且观察组各指标低于对照组,差异有统计学意义(P<0.05).干预后,观察组血糖达标时间短于对照组,且胰岛素用量及低血糖人均次数低于对照组,差异有统计学意义(P<0.05).干预前,2组 MAGE、SDBG、MODD水平比较,差异无统计学意义(P>0.05).干预后,2组 MAGE、SDBG、MODD水平低于干预前,且观察组各指标低于对照组,差异有统计学意义(P<0.05).干预前,2组血糖达标率及KAP各维度评分比较,差异无统计学意义(P>0.05).干预后,2组血糖达标率及 KAP各维度评分高于干预前,且观察组各指标高于对照组,差异有统计学意义(P<0.05).结论 FGM 系统可有效改善成人糖尿病患者血糖生化指标,缩短血糖达标时间,降低胰岛素用量及低血糖发生风险,并能提升患者KAP水平.

Objective To investigate the effect of flash glucose monitoring(FGM)system on glycemic control and knowledge-attitude-practice(KAP)levels in adult diabetic patients.Methods A total of 230 dia-betic patients admitted to our hospital from January 2022 to January 2024 were selected and randomly divided into observation group and control group,with 115 cases in each group.The control group was treated with fingertip blood glucose detection and insulin pump,while the observation group was treated with FGM com-bined with insulin pump.The blood glucose biochemical indexes[fasting blood glucose(FPG),2-hour post-prandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c)],blood glucose fluctuation indexes[average blood glucose fluctuation range(MAGE),blood glucose standard deviation(SDBG),blood glucose daily aver-age difference(MODD)],as well as blood glucose reaching the standard time,insulin dosage,hypoglycemia per capita times,blood glucose reaching the standard rate,and KAP score were compared between the two groups.Results There was no significant difference in the levels of HbA1c,FPG and 2 hours FPG between the two groups before intervention(P>0.05).After the intervention,the levels of HbA1c,FPG and 2hPG in the two groups were lower than those before the intervention,and the indexes in the observation group were lower than those in the control group,with statistical significance(P<0.05).After the intervention,the time to reach the standard of blood sugar in the observation group was shorter than that in the control group,and the insulin dosage and hypoglycemia per capita times were lower than those in the control group,with statistical significance(P<0.05).Before the intervention,the levels of MAGE,SDBG and MODD between the two groups were not statistically significant(P>0.05).After the intervention,the levels of MAGE,SDBG and MODD in the two groups were lower than those before the intervention,and the indexes in the observation group were lower than those in the control group,with statistical significance(P<0.05).Before the interven-tion,there was no significant difference between the two groups in the blood glucose compliance rate and KAP scores(P>0.05).After the intervention,the blood glucose compliance rate and KAP scores in the two groups were higher than those before the intervention,and the indexes in the observation group were higher than those in the control group,with statistical significance(P<0.05).Conclusion The FGM system can effective-ly improve glycemic biochemical indicators,shorten the time to reach glycemic target,reduce insulin dosage and the risk of hypoglycemia,and enhance the KAP level in adult diabetic patients.
