目的:探讨血清微小 RNA-135a-5p(miR-135a-5p)、同源框基因 A10(HOXA10)水平与冻融胚胎移植(FET)患者子宫内膜容受性指标和妊娠结局的相关性.方法:选取2022 年4 月至2024 年4 月于郑州大学第二附属医院生殖医学部行FET 的130例患者为研究对象,依据最终妊娠结局分为妊娠组和未妊娠组.采用 qRT-PCR 法检测血清 miR-135a-5p 水平,采用 ELISA 法检测血清 HOXA10 水平,采用超声诊断仪检测子宫内膜容受性指标内膜厚度(ET)、内膜体积(EV)、阻力指数(RI)和搏动指数(PI).采用Pearson 和 Spearman 相关性分析评估指标相关性;采用 logistic 回归分析筛选影响因素;采用 ROC 曲线分析指标对妊娠结局的预测价值,采用 DeLong 检验比较曲线下面积(AUC)值.结果:未妊娠组的血清 miR-135a-5p 水平及 ET、RI、PI 高于妊娠组,HOXA10 水平及EV 低于妊娠组(P<0.05).FET 患者血清 miR-135a-5p 水平与 ET、RI、PI 呈正相关,与EV 呈负相 关(P<0.05);HOXA10 水平与 ET、RI、PI 呈负相关,与 EV 呈正相关(P<0.05).miR-135a-5p 升高、ET 升高、HOXA10 降低、EV 降低是 FET 患者未妊娠的独立危险因素(P<0.05).miR-135a-5p、HOXA10、ET、EV 单独及四者联合预测的 AUC 分别为0.792、0.800、0.770、0.780、0.928,四者联合的 AUC 较高(Z=2.915、2.795、3.326、3.060,P<0.05).结论:miR-135a-5p、HOXA10 与子宫内膜容受性密切相关,其与指标ET、EV 联合检测可有效预测 FET 患者妊娠失败.

Objective:To explore the correlation between serum levels of microRNA-135a-5p(miR-135a-5p)and homeobox gene A10(HOXA10)with endometrial receptivity in-dicators and pregnancy outcomes in patients undergoing frozen-thawed embryo transfer(FET).Methods:From April 2022 to April 2024,130 patients who underwent FET in the Department of Reproductive Medicine of our hospital were enrolled as the study subjects.They were divided into pregnancy group and non pregnancy group based on the final pregnancy outcome.The ser-um levels of miR-135a-5p was detected by qRT-PCR.The serum levels of HOXA10 was detec-ted by ELISA.Endometrial receptivity indicators,including endometrial thickness(ET),endom-etrial volume(EV),resistant index(RI),and pulsatility index(PI)were measured by ultra-sound.Pearson and spearman correlation analysis were used to evaluate the correlation between indicators.Logistic regression analysis was performed to identify influencing factors.The predic-tive value of indicators for pregnancy outcomes was analyzed by receiver operating characteristic(ROC)curves,and the AUC values were compared using the DeLong test.Results:Compared with the pregnancy group,the non pregnancy group had higher serum miR-135a-5p level,ET,RI,and PI,but lower HOXA10 level and EV(P<0.05).In FET patients,serum miR-135a-5p was positively correlated with ET,RI,and PI,and negatively correlated with EV(P<0.05).Conversely,serum HOXA10 was negatively correlated with ET,RI,and PI,and positively corre-lated with EV(P<0.05).Elevated miR-135a-5p,elevated ET,decreased HOXA10,and de-creased EV were independent risk factors for non pregnancy in FET patients(P<0.05).The AUC values for predicting pregnancy outcome by miR-135a-5p,HOXA10,ET,and EV alone and their combination were 0.792,0.800,0.770,0.780,and 0.928,respectively.The AUC of the four-marker combination was significantly higher than that of any single marker(Z=2.915,2.795,3.326,3.060,P<0.05).Conclusion:Serum levels of miR-135a-5p and HOXA10 are closely associated with endometrial receptivity.Their combination with ET and EV can effective-ly predict pregnancy failure in FET patients.
