基于营养风险评估的个体化营养支持对食管癌术后辅助化疗患者营养指标和免疫水平的影响OA
Impact of individualized nutritional support based on nutritional risk assessment on nutritional indicators and immune levels of esophageal cancer patients undergoing postoperative adjuvant chemotherapy
目的 探讨基于营养风险评估的个体化营养支持对食管癌术后辅助化疗患者营养指标和免疫水平的影响.方法 选取2024 年 1 月至 2024 年 10 月于上海市胸科医院治疗的 120 例食管癌术后辅助化疗患者,采用便利抽样法进行研究,并通过随机数字表法分为对照组和观察组,各 60 例.对照组给予常规营养支持治疗,观察组给予基于营养风险评估的个体化营养支持治疗.比较两组患者干预前后营养指标[体质量指数(body mass index,BMI)、血清白蛋白(albumin,ALB)和血红蛋白(hemoglobin,Hb)]、生活质量和免疫水平(血清 CD3+、CD4+和 CD4+/CD8+)的变化情况,以及干预期间的不良反应情况.结果 在营养指标水平、生活质量量表(QLQ-C30)中功能分量表评分和免疫水平方面,两组干预前比较,差异均无统计学意义(均P>0.05),干预后较干预前均升高(均 P<0.05),且观察组升高均更明显(均 P<0.05).干预期间,观察组不良反应发生率均低于对照组(均P<0.05).结论 对于食管癌术后辅助化疗患者,实施基于营养风险评估的个体化营养支持的疗法,有助于提升患者营养指标,提高生活质量和免疫功能,降低不良反应发生风险.
Objective To explore the impact of individualized nutritional support based on nutritional risk assessment on the nutritional indicators and immune levels of esophageal cancer patients undergoing postoperative adjuvant chemotherapy.Methods A total of 120 esophageal cancer patients with postoperative adjuvant chemotherapy in Shanghai Chest Hospital from January to October 2024 were se-lected and studied by convenience sampling method.They were divided into the control group and the observation group through random number table method,with 60 cases in each group.The control group received routine nutritional support treatment,while the observation group received individualized nutritional support treatment based on nutritional risk assessment.The changes of nutritional indicators including body mass index(BMI),serum albumin(ALB),and hemoglobin(Hb),immune levels including serum CD3+,CD4+,and CD4+/CD8+,and the quality of life before and after the intervention,and adverse reactions during the intervention were compared between the two groups.Results The levels of all nutritional indicators,the scores of the functional components of the quality of life scale QLQ-C30,and the immune levels were not significantly different between the two groups before the intervention(all P>0.05),were all increased in both groups after the intervention(all P<0.05),and the increases in the observation group were all more obvious than those in the control group(all P<0.05).During the intervention,the incidences of adverse reactions in the observation group were all lower than those in the control group(all P<0.05).Conclusions The implementation of individualized nutritional support therapy based on nutritional risk assessment can effectively improve the nutritional indicators of patients,enhance the quality of life and immune function,and reduce the risk of adverse reactions in esophageal cancer patients undergoing postoperative adjuvant chemotherapy.
