血管内碎石术与冠状动脉旋磨术预处理对冠状动脉重度狭窄合并钙化结节患者影响的对比研究OA

Objective To compare the effectiveness of intravascular lithotripsy(IVL)and rotational atherectomy(RA)preconditioning in patients with severe coronary stenosis and calcified nodules.Methods A total of 172 patients with severe coronary stenosis and calcified nodules admitted to People's Hospital of Xinjiang Uygur Autonomous Region from January 2023 to June 2024 were selected as the research subjects,and patients were divided into the IVL group(n=73),and the RA group(n=99)based on the surgical approach.The patients in the IVL group received IVL,the patients in the RA group received RA.The minimum lumen area and lumen area at calcified nodules before and after surgery,immediate acquisition of lumen area after surgery,incidence of intraoperative complications,major adverse cardiovascular events(MACE)during follow-up were compared between the two groups.Results The minimum lumen area and lumen area at calcified nodules after surgery,and immediate acquisition of lumen area after surgery in the IVL group were larger than those in the RA group(P<0.05).There was no statistically significant difference in the total intraoperative complication rate between the two groups(P>0.05).The cumulative MACE free survival rate during follow-up in the IVL group was higher than that in the RA group(P<0.05).Conclusion Compared with RA preconditioning,IVL preconditioning can increase the minimum lumen area and lumen area at calcified nodules after surgery,and immediate acquisition of lumen area after surgery in patients with severe coronary stenosis and calcified nodules,and improve cumulative MACE free survival rate.
