目的 探究蓝萼甲素(GLA)对腰椎间盘突出(LDH)重吸收的作用及其分子机制.方法 将人单核细胞白血病细胞THP-1分为对照组、PMA诱导组、5 μmol/L GLA组、10 μmol/L GLA组和20 μmol/L GLA组,采用免疫荧光染色检测各组细胞M1/M2极化水平.各组细胞与人髓核细胞进行Transwell共培养实验,采用CCK-8实验检测髓核细胞增殖活性,TUNEL染色实验检测髓核细胞凋亡.采用自体髓核移植法建立LDH大鼠模型,分为LDH组、GLA组(5 mg/kg GLA)和激活剂组(5 mg/kg GLA+5 mg/kg PMA),未建模大鼠设为对照组,每组10只.末次给药1 d后,采用Von Frey纤维丝法测定其机械痛阈值,采用ELISA法检测血清TNF-α、IL-1β、IL-6和IL-10水平,HE和Masson染色检测大鼠椎间盘组织病理学和形态学变化,免疫荧光染色检测巨噬细胞M1/M2极化,Western blot检测MMP-3、TIMP-1、VEGF、COL Ⅱ、p-NF-κB p65、NF-κB p65、p-IκBα和IκBα蛋白表达水平.结果 与对照组比较,PMA诱导组髓核细胞凋亡率、iNOS阳性率和iNOS+/Arg1+比值均升高(P<0.05),髓核细胞增殖活力和Arg1阳性率均降低(P<0.05);与PMA诱导组比较,5,10,20 μmol/L GLA组髓核细胞凋亡率、iNOS阳性率和iNOS+/Arg1+比值均降低(P<0.05),髓核细胞增殖活力和Arg1阳性率均升高(P<0.05).与LDH组比较,GLA组大鼠椎间盘结构破坏减轻,组织病理评分降低(P<0.05),大鼠椎间盘组织重吸收质量、机械痛阈值、IL-10水平、COLⅡ、TIMP-1蛋白水平和Arg1阳性率均升高(P<0.05),TNF-α、IL-1β和IL-6水平、MMP-3和VEGF蛋白水平、iNOS阳性率以及MMP-3/TIMP-1、iNOS+/Arg1+、p-NF-κB p65/NF-κB p65和p-IκBα/IκBα比值均降低(P<0.05);激活剂逆转了GLA对椎间盘突出重吸收的改善作用.结论 蓝萼甲素可能通过抑制NF-κB信号通路激活,激活巨噬细胞M2型极化,抑制炎性因子分泌和髓核细胞凋亡,促进髓核细胞外基质合成,进而推动突出椎间盘组织的重吸收.

Objective To investigate the effect of glaucocalyxin A(GLA)on the resorption of lumbar disc herniation(LDH)and its molecular mechanisms.Methods The human monocytic leukemia cells THP-1 were divided into control group,PMA induction group,5 μmol/L GLA group,10 μmol/L GLA group and 20 μmol/L GLA group.The polarization levels of M1/M2 in cells were detected by immunofluorescence staining.Cells in each group were co-cultured with human nucleus pulposus cells in Transwell experiments.The proliferation activity of nucleus pulposus cells was detected by CCK-8 assay,and the apoptosis of nucleus pulposus cells was detected by TUNEL staining.The LDH rat model was established by autologous nucleus transplantation.The LDH rats were divided into LDH group,GLA(5 mg/kg)group and activator group(5 mg/kg GLA+5 mg/kg PMA),with 10 rats in each group,and ten unmodeled rats were set as control group.One day after the last administration,the mechanical pain threshold was determined by the Von Frey filament method,the levels of TNF-α,IL-1β,IL-6 and IL-10 in serum were detected by ELISA,the pathological and morphological changes of rat intervertebral disc tissues were detected by HE and Masson staining,the polarization of macrophages M1/M2 was detected by immuno-fluorescence staining,and the protein expression levels of MMP-3,TIMP-1,VEGF,COL Ⅱ,p-NF-κB p65,NF-κB p65,p-IκBα and IκBα were detected by Western blot.Results Compared with control group,the apoptosis rate of nucleus pulposus cells,the positive rate of iNOS,and the ratio of iNOS+/Arg1+were increased in PMA induction group(P<0.05),while the proliferation activity of nucleus pulposus cells and the positive rate of Arg1 were decreased(P<0.05).Compared with PMA induction group,the apoptosis rate of nucleus pulposus cells,the positive rate of iNOS,and the ratio of iNOS+/Arg1+in 5,10,20 μmol/L GLA groups were all decreased(P<0.05),and the proliferation activity of nucleus pulposus cells and the positive rate of Arg1 were all increased(P<0.05).Compared with LDH group,the damage of intervertebral disc structure was alleviated,the tissue pathological score was decreased in GLA group(P<0.05),the resorption of intervertebral disc tissue,mechanical pain threshold,IL-10 level,protein levels of COLⅡ and TIMP-1,and Arg1 positive rate were all increased(P<0.05),and the levels of TNF-α,IL-1β and IL-6,protein levels of MMP-3 and VEGF,iNOS positive rate,and the ratios of MMP-3/TIMP-1,iNOS+/Arg1+,p-NF-κB p65/NF-κB p65 and p-IκBα/IκBα were all decreased(P<0.05).The activator PMA reversed the improvement effect of GLA on the resorption of intervertebral disc protrusion.Conclusion GLA may promote the resorption of herniated intervertebral disc tissues by inhibiting the activation of NF-κB signaling pathway,activating M2-type polarization of macrophages,inhibiting the secretion of inflammatory factors and the apoptosis of nucleus pulposus cells,and promoting the synthesis of extracellular matrix of nucleus pulposus cells.
