基于"亢害承制"理论探讨雄激素性脱发雄激素异常作用的因机证治OA

Androgenetic alopecia is the most common type of hair loss disease in clinical practice.Abnormal androgen effect is the key factor leading to androgenetic alopecia.The theory of"harmful hyperactivity and responding inhibition"is the fundamental principle for understanding human physiology and pathology,as well as for guiding syndrome differentiation and treatment in clinical practice.The mechanism of abnormal androgen effect in androgenetic alopecia can be understood according to the theory of"hyperactivity leads to harm,commitment brings the control".The deficiency of the liver and kidney results in inadequate restraint,leading to the inability to maintain the homeostatic balance of androgen metabolism in scalp hair follicle cells,and it is considered the initiating factor of androgenetic alopecia.With the affection of other internal organs,the transformation of Qi is dysfunctional and the distribution of essence,blood,and body fluids is hindered.It causes pathological products such as blood stasis,retained fluid and turbid phlegm to obstruct the hair orifices.The excess pathogenic factors manifest as the accumulation of inflammatory mediators and reactive oxygen species,which further aggravate androgenetic alopecia.The interplay of both aspects creates the vicious cycle,which is the key reason why androgenetic alopecia tends to be chronic and difficult to cure.Based on the pathological mechanism of abnormal androgen effect,this paper proposes the treatment principle of"restraining excess and alleviating hyperactivity".According to the disease pathogenesis,the interventions with traditional Chinese medicine are applied to suppress androgen activity,activate Wnt/β-catenin pathway and eliminate inflammatory mediators and reactive oxygen species for the improvement of follicular function.It provides practical value for the prevention and treatment of androgenetic alopecia.
