多囊卵巢综合征(PCOS)是育龄期女性常见的内分泌代谢性疾病,临床以高雄激素血症、排卵障碍及卵巢多囊样改变为主要特征,常伴随胰岛素抵抗和慢性低度炎症.PCOS常伴随月经失调、体质量异常增加及皮肤病变等临床表现,严重影响患者的生活质量及生殖健康.近年来,针对PCOS的治疗多采用雄激素受体抑制剂、芳香化酶抑制剂、口服避孕药以及胰岛素敏感剂等,虽然在疾病的疗效上取得一定进展,却依然出现胃肠道不适、激素水平波动、体质量变化以及肝肾损伤等副作用.中药黄酮类化合物可通过抗炎、抗氧化、抑制细胞自噬和凋亡、改善胰岛素抵抗(IR)、缓解高雄激素血症(HA)、调节脂质代谢以及肠道菌群等多种机制来改善PCOS,对提高患者的生活质量、恢复生殖功能以及减轻相关症状具有积极作用.大量研究表明,中药黄酮类化合物能够通过调控磷脂酰肌醇3-激酶(PI3K)/蛋白激酶B(Akt)、丝裂原活化蛋白激酶(MAPK)、核转录因子-κB(NF-κB)、腺苷酸活化蛋白激酶(AMPK)、核因子E2相关因子2(Nrf2)、转化生长因子β1(TGF-β1)/Smad以及Janus激酶(JAK)/转录激活因子(STAT)等信号通路,调节PCOS相关蛋白的转录和表达,改善PCOS的激素水平、代谢状态和炎症反应.该研究通过系统分析近年国内外文献,重点探讨了中药黄酮类化合物调控PCOS的分子机制及其作用靶点,旨在为PCOS的发病机制研究、新型药物开发及临床应用提供理论依据.

Polycystic ovary syndrome(PCOS)is a common endocrine metabolic disease in women of reproductive age,characterized by hyperandrogenemia,ovulation disorders,and polycystic ovarian changes,and often accompanied by insulin resistance and chronic low-grade inflammation.PCOS is often accompanied by menstrual dysfunction,abnormal increase in body mass,and skin lesions,which have a serious impact on the quality of life and reproductive health of patients.In recent years,androgen receptor inhibitors,aromatase inhibitors,oral contraceptives,and insulin sensitizers have been used in the treatment of PCOS.Although some progress has been made in terms of the efficacy of the disease,gastrointestinal discomforts,fluctuating hormone levels,body mass changes,and liver and kidney injuries are still side effects.Flavonoids from traditional Chinese medicine can improve PCOS through various mechanisms such as anti-inflammatory,antioxidant,inhibition of cell autophagy and apoptosis,improvement of insulin resistance(IR),alleviation of hyperandrogenemia(HA),regulation of lipid metabolism,and modulation of intestinal flora,etc.,which can improve the life quality of patients,restore the reproductive function,and alleviate the related symptoms.Numerous studies have shown that flavonoids from traditional Chinese medicine can regulate the transcription and expression of PCOS-related proteins,and improve the hormone level,metabolic status and inflammatory response in PCOS.by modulating signaling pathways such as regulating phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt),mitogen-activated protein kinase(MAPK),nuclear transcription factor-κB(NF-κB),adenylic acid-activated protein kinase(AMPK),nuclear factor E2-related factor 2(Nrf2),transforming growth factor β1(TGF-β1)/Smad,and Janus kinase(JAK)/activator of transcription(STAT).Through systematic analysis of recent domestic and international literature,this study focuses on the molecular mechanisms and targets of flavonoids in traditional Chinese medicine in regulating PCOS,aiming to provide a theoretical basis for the study of the pathogenesis of PCOS,the development of new drugs and their clinical applications.
