三合宁嗽汤治疗肺脾气虚型肺部术后慢性咳嗽临床观察OA
Clinical study on Sanhe Ningsou Decoction in treating chronic cough after surgery(lung and spleen qi deficiency pattern)
目的:评价三合宁嗽汤联合复方甲氧那明胶囊治疗肺部手术后慢性咳嗽(肺脾气虚型)的临床疗效.方法:选取2024年1月—10月安徽中医药大学第一附属医院收治的60例肺部术后慢性咳嗽(肺脾气虚型)患者,根据治疗方案分为治疗组(30例)与对照组(30例).对照组口服复方甲氧那明胶囊,治疗组在对照组治疗基础上口服三合宁嗽汤,药物组成:麦冬15 g,半夏10 g,柴胡10 g,黄芩10 g,人参10 g,僵蚕9 g,蝉蜕9 g,片姜黄9 g,炒大黄12 g,生姜10 g,大枣10 g.两组均治疗4周.主要观察指标包括治疗前后中文版莱斯特咳嗽问卷(LCQ-MC)评分、咳嗽症状积分、中医证候积分,并评价临床疗效.结果:治疗后,两组患者LCQ-MC评分的生理、心理、社会维度及总分均较治疗前明显升高,咳嗽症状积分及中医证候积分均较治疗前明显降低,且治疗组各项指标优于对照组,差异有统计学意义(P<0.05,P<0.01).治疗组总有效率明显高于对照组(73.3%vs 40.0%),不良反应发生率低于对照组,差异有统计学意义(P<0.05).结论:三合宁嗽汤联合复方甲氧那明胶囊能显著改善肺部术后慢性咳嗽(肺脾气虚型)患者的咳嗽症状及生活质量,提高临床疗效,显示出中西医结合治疗的优势.
Objective:To evaluate the clinical efficacy of Sanhe Ningsou Decoction(Three Mixtures Cough-Quieting Decoction)combined with compound methoxyphenamine capsule in treating chronic cough after lung surgery(lung-spleen qi deficiency pattern).Methods:A total of 60 patients with chronic cough after lung surgery(lung-spleen qi deficiency pat-tern)admitted to the First Affiliated Hospital of Anhui University of Chinese Medicine from January to October 2024 were selected and divided into a treatment group(30 cases)and a control group(30 cases).The control group received oral com-pound methoxyphenamine capsule,while the treatment group received oral Sanhe Ningsou Decoction in addition to the con-trol treatment.The composition of Sanhe Ningsou Decoction included Maidong 15 g,Banxia 10 g,Chaihu 10 g,Huangqin 10 g,Renshen 10 g,Jiangcan 9 g,Chantui 9 g,Pianjianghuang 9 g,Chaodahuang 12 g,Shengjiang 10 g,and Dazao 10 g.Both groups were treated for 4 weeks.The main observation indicators included the scores of the Chinese version of the Leicester Cough Questionnaire(LCQ-MC),cough symptom score,and traditional Chinese medicine(TCM)syndrome score before and after treatment,and clinical efficacy was evaluated.Results:After treatment,the physiological,psychological,and social di-mensions and total score of LCQ-MC in both groups were significantly increased compared with before treatment,while the cough symptom score and TCM syndrome score were significantly decreased.The treatment group showed better improve-ments in all indicators than the control group,with statistically significant differences(P<0.05,P<0.01).The total effective rate in the treatment group was significantly higher than that in the control group(73.3%vs.40.0%),and the incidence of adverse reactions was lower in the treatment group,with statistically significant differences(P<0.05).Conclusion:Sanhe Ningsou Decoction combined with compound methoxyphenamine capsule can significantly improve cough symptoms and quality of life in patients with chronic cough after lung surgery(lung-spleen qi deficiency pattern),enhance clinical efficacy,and demonstrate the advantages of integrated Chinese and Western medicine therapy.
