针药结合治疗肝肾亏虚型腰椎间盘突出症临床疗效观察OA
Clinical efficacy observation of acupuncture combined with Chinese herbal medicine in treating lumbar disc herniation of liver-kidney deficiency pattern
目的:观察对药经验方联合"强肾通督"针法治疗肝肾亏虚型腰椎间盘突出症(LDH)的临床疗效,为其中医综合治疗提供科学依据与实践参考.方法:选取2025年1月—2025年10月山西省针灸医院针灸二科门诊及住院诊断为LDH肝肾亏虚型的患者74例,采用随机数字表法按1∶1比例分为治疗组与对照组[1],每组37例.两组均给予基础治疗,对照组患者予以对药经验方(熟地黄10 g,细辛3 g,牛膝10 g,鸡血藤10 g,威灵仙10 g,桑寄生10 g,独活10 g,砂仁10 g)补益肝肾、舒筋通络、祛瘀止痛,治疗组在对照组治疗的基础上予以"强肾通督"针法(主穴选取肾俞、命门、委中,配穴选取后溪、环跳、阳陵泉、承山、悬钟)疏通经络、强肾健骨.以临床疗效、腰椎功能评分(JOA)、视觉模拟量表(VAS)疼痛评分为观察指标.结果:治疗前,两组患者JOA、VAS评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后两组患者JOA、VAS评分均得到改善,且治疗组JOA、VAS评分优于对照组,治疗组总有效率(94.59%,35/37)高于对照组(81.08%,30/37),差异均有统计学意义(P<0.05,P<0.01).结论:对药经验方联合"强肾通督"针法治疗LDH肝肾亏虚型临床疗效确切,值得应用.
Objective:To observe the clinical efficacy of a paired-herb empirical formula combined with the"Kidney-Strengthening and Du Mai-Unblocking"acupuncture method in treating lumbar disc herniation(LDH)of the liver-kidney defi-ciency pattern,and to provide a scientific basis and practical reference for its integrated traditional Chinese medicine treat-ment.Methods:A total of 74 patients diagnosed with LDH of the liver-kidney deficiency pattern in outpatient and inpatient settings of the Second Department of Acupuncture and Moxibustion at Shanxi Acupuncture Hospital from January 2025 to Oc-tober 2025 were selected.They were randomly divided into a treatment group and a control group at a 1:1 ratio using a random number table method,with 37 cases in each group.Both groups received basic treatment.Patients in the control group were addi-tionally treated with a paired-herb empirical formula(Shudihuang 10 g,Xixin 3 g,Niuxi 10 g,Jixueteng 10 g,Weilingxian 10 g,Sangjisheng 10 g,Duhuo 10 g,and Sharen 10 g)to tonify the liver and kidney,relax the sinews and unblock the meridians,and dispel stasis and alleviate pain.On the basis of the control group's treatment,patients in the treatment group also received the"Kidney-Strengthening and Du Mai-Unblocking"acupuncture method[main acupoints:Shenshu(BL23),Mingmen(GV4),Weizhong(BL40);adjunct acupoints:Houxi(SI3),Huantiao(GB30),Yanglingquan(GB34),Chengshan(BL57),and Xuan-zhong(GB39)]to unblock the meridians,strengthen the kidney,and fortify the bones.The observed outcomes included clinical efficacy,Japanese Orthopaedic Association(JOA),and Visual Analog Scale(VAS)pain scores.Results:Before treatment,there were no statistically significant differences in JOA and VAS scores between the two groups(P>0.05),indicating compa-rability.After treatment,both groups showed improvement in JOA and VAS scores.The treatment group had significantly bet-ter JOA and VAS scores than the control group.The total effective rate in the treatment group(94.59%,35/37)was higher than that in the control group(81.08%,30/37).The differences were statistically significant(P<0.05,P<0.01).Conclusion:The paired-herb empirical formula combined with the"Kidney-Strengthening and Du Mai-Unblocking"acupuncture method has a definite clinical effect in treating LDH of the liver-kidney deficiency pattern and is worthy of clinical application.
