目的 探讨重型颅脑损伤(STBI)患者血清神经调节蛋白-1(NRG-1)、富亮氨酸α-2糖蛋白-1(LRG1)、血管性血友病因子裂解蛋白酶13(ADAMTS13)表达及其对预后的预测价值.方法 纳入2022年2月至2024年6月我院收治的140例STBI患者作为观察对象(STBI组).根据GOS评分分为预后良好组(89例)和预后不良组(51例).同期选取我院体检中心140例健康志愿者为对照组.采用酶联免疫吸附法检测血清NRG-1、LRG1、ADAMTS13水平.采用Lo-gistic 回归分析影响STBI患者预后不良的相关因素.ROC曲线分析血清NRG-1、LRG1、ADAMTS13单独及联合对STBI患者预后不良的预测价值.结果 STBI组血清NRG-1、ADAMTS13水平低于对照组(P<0.05),LRG1水平高于对照组(P<0.05).140例STBI患者中,其中51例预后不良,89例预后良好.单因素Logistic分析显示,血清NRG-1、AD-AMTS13、LRG1水平与入院时GCS评分、合并脑疝为STBI患者预后不良影响因素(P<0.05).多因素Logistic分析显示,LRG1为STBI患者预后不良的危险因素,入院时GCS评分、NRG-1、ADAMTS13是STBI患者预后不良的保护因素(P<0.05).血清 NRG-1、LRG1、ADAMTS13 三者联合预测 AUC 值大于单一指标(Z=4.354,Z=3.628,Z=4.210,P<0.05).结论 STBI患者血清NRG-1、ADAMTS13水平降低,LRG1水平升高,对STBI患者预后不良具有预测价值.

Objective To explore the expression of serum neuregulin-1(NRG-1),leucine rich alpha-2 glycoprotein 1(LRG1),and a disintegrin-like and metalloproteinase with thrombospondin type Ⅰ motif 13(ADAMTS13)in patients with severe traumatic brain injury(STBI)and their predictive value for prognosis.Methods From February 2022 to June 2024,140 patients with STBI admitted in our hospital were included as the observation subjects(STBI group).According to the GOS score,they were classified into a good prognosis group(89 cases)and a poor prognosis group(51 cases).Meanwhile,140 healthy volunteers from the physical examination center of our hospital were labeled as the control group.Enzyme linked immunosorbent assay was per-formed to detect serum NRG-1,LRG1,and ADAMTS13.Logistic regression was used to explore the factors related to poor prognosis in STBI patients.ROC curve was performed to explore the predictive value of serum NRG-1,LRG1,ADAMTS13 alone and their joint for poor prognosis in STBI patients.Results The STBI group had lower serum NRG-1 and ADAMTS13(P<0.05),and higher LRG1 than the control group(P<0.05).Of the 140 patients with STBI,51 of them had poor prognosis and 89 had good prognosis.Univariate Logistic regression analysis showed that serum NRG-1,ADAMTS13,and LRG1 levels with GCS score at ad-mission,and combined brain herniation were the influencing factors for poor prognosis in STBI patients(P<0.05).Multivariate Logistic regression analysis showed that LRG1 was an independent risk factor for poor prognosis in STBI patients,whereas GCS score at admission,NRG-1,and ADAMTS13 were independent protective factors(P<0.05).The AUC value of the combined de-tection of NRG1,LRG1,and ADAMTS13 was significantly higher than that of any single indicator(Z=4.354,Z=3.628,Z=4.210,P<0.05).Conclusion Serum levels of NRG1 and ADAMTS13 are decreased,while LRG1 level is increased in patients with STBI.These indicators have predictive value for poor prognosis in STBI patients.
