目的 探究CEACAM19调节PI3K/Akt通路对胃癌免疫微环境的改善作用.方法 以RT-qPCR检测胃癌患者癌组织及其配对癌旁组织中CEACAM19表达,分析与其临床病理参数间的相关性.HGC-27细胞用CEACAM19 siR-NA和空载慢病毒质粒分别干预后制备荷瘤小鼠模型,随机分为对照组、CEACAM19敲低组、空载组、CEACAM19敲低+740 Y-P组,用PI3K激活剂740 Y-P干预后检测肿瘤体积与质量;以RT-qPCR和免疫印迹法检测CEACAM19与PI3K/Akt表达;流式细胞实验检测外周血免疫细胞的分化;ELISA检测血清免疫细胞因子水平;CCK-8法检测免疫微环境中NK细胞、巨噬细胞杀伤力.结果 CEACAM19表达水平与胃癌患者淋巴结转移、侵袭深度和临床分期相关.与对照组比较,CEACAM19敲低组CD4+与CD8+T细胞比例、M1型巨噬细胞比例、IL-6及IL-2水平、NK细胞与巨噬细胞杀伤力升高(P<0.05),M2型巨噬细胞比例、CEACAM19表达、p-PI3K/PI3K及p-Akt/Akt、肿瘤体积与质量、TGF-β1及IL-10水平降低(P<0.05).740 Y-P可减弱CEACAM19敲低对胃癌小鼠上述各指标的作用.结论 敲低CEACAM19可抑制PI3K/Akt信号通路激活,进而调控胃癌免疫微环境,增强免疫细胞对肿瘤细胞杀伤力,可作为胃癌的新型标志物.

Objective To investigate the role of CEACAM19 regulating the PI3K/Akt pathway in improving the immune microenvironment of gastric cancer.Methods The CEACAM19 was detected in gastric cancer tissues and paired adjacent tissues using RT-qPCR,and its correlation with clinical pathological parameters was analyzed.HGC-27 cells were treated with CE AC AM1 9 siRNA or empty lentiviral vector to establish tumor-bearing mouse models,which were randomly divided into control group,CEACAM19 knockdown group,empty vector group,and CEACAM19 knockdown+740 Y-P group.Tumor volume and mass were measured after intervention with PI3K activator 740 Y-P.CEACAM19 and PI3K/Akt were detected by RT-qPCR and immunoblot-ting.Peripheral blood immune cell differentiation was assessed by flow cytometry.Serum immune cytokine were measured by ELISA.NK cell and macrophage cytotoxicity in the immune microenvironment were evaluated using the CCK-8 assay.Results CEACAM19 were associated with lymph node metastasis,invasion depth,and clinical stage in gastric cancer patients.Compared with the control group,the CEACAM19 knockdown group showed increased proportions of CD4+and CD8+T cells,M1-type mac-rophages,IL-6 and IL-2 levels,and NK cell and macrophage cytotoxicity(P<0.05),while decreased proportions of M2-type mac-rophages,CEACAM19,p-PI3K/PI3K and p-Akt/Akt,tumor volume and mass,and TGF-β1 and IL-10(P<0.05).740 Y-P atten-uated the effects of CEACAM19 knockdown on the aforementioned indicators in gastric cancer mice.Conclusion CEACAM19 knockdown could inhibit the activation of the PI3K/Akt signaling pathway,thereby modulating the immune microenvironment of gastric cancer and enhancing the cytotoxicity of immune cells against tumor cells,making it a potential novel biomarker for gastric cancer.
