囊型/泡型肝棘球蚴病灶PDGFs表达水平及对肝纤维化程度的影响OA

Objective To investigate the expression levels of platelet-derived growth factors(PDGFs)in cystic/alveolar echinococcosis lesions and their association with liver fibrosis.Methods Retrospective analysis the clinical data of 89 patients with hepatic echinococcosis who underwent surgical treatment in our hospital from December 2021 to December 2024.According to the pathological diagnosis results,the patients were divided into the cystic hepatic echinococcosis group(cystic group)and the alveolar hepatic echinococcosis group(alveolar group).The degree of hepatic fibrosis in patients was evaluated by the Knodell fi-brosis score.The cyst wall and surrounding liver tissues of patients with cystic echinococcosis,the central part,marginal part of the lesions and distant normal liver tissues of patients with alveolar echinococcosis were collected.mRNA and protein levels of PDGFs(PDGF-AA,PDGF-BB,PDGF-AB,PDGF-CC,PDGF-DD)in each tissue sample were determined by real-time fluorescence quanti-tative PCR and Western Blot.Differences in the levels of PDGFs and the degree of fibrosis among different groups of patients and different tissues were compared,and the correlation between the levels of PDGFs and hepatic fibrosis was evaluated by Spearman correlation test.Results Among 89 patients with hepatic echinococcosis enrolled,there were 49 males and 40 females,with an av-erage age of(45.05±7.83)years.There were 76 patients in cystic group and 13 patients in alveolar group.The degree of hepatic fibrosis in the alveolar group was more severe than that in the cystic group(Z=2.358,P=0.018).The relative expression levels of mRNA and protein of PDGF-AA,PDGF-BB,PDGF-AB,PDGF-CC,PDGF-DD in the marginal tissues of the cyst wall of patients in the cystic group and the alveolar group were significantly higher than those in the normal liver tissues(cystic group:t=30.375,30.331,27.444,38.124,30.137,39.750,39.157,32.548,35.086,31.616,all P＜0.001;alveolar group:t=16.203,7.123,13.834,14.021,11.607,11.338,6.140,6.059,20.906,7.430,all P＜0.001).The relative expression levels of mRNA and pro-tein of PDGF-BB,PDGF-AB,PDGF-DD in the lesion sites of patients with CE were significantly lower than those in the alveolar group(t=-2.210,-3.117,-3.450,-2.202,-2.632,-3.420,P=0.030,0.002,0.001,0.030,0.010,0.001).In all pa-tients,patients in the cystic group and patients in the alveolar group,the relative expression levels of mRNA and protein of PDGF-BB,PDGF-AB,PDGF-DD in the lesion tissues were positively correlated with the Knodell fibrosis score(P＜0.05).Con-clusion The degree of hepatic fibrosis in patients with alveolar hepatic echinococcosis is significantly more severe than that in pa-tients with cystic hepatic echinococcosis.The protein and transcriptional levels of PDGF-BB,PDGF-AB,PDGF-DD are also higher than those in patients with cystic echinococcosis.The above PDGF subtypes are significantly positively correlated with the degree of hepatic fibrosis in patients with hepatic echinococcosis.
