目的 对比3D可视化技术和CT平扫三维重建在改良S.T.O.N.E评分,S.O.L.V.E评分以及SHA.LIN评分对于预测输尿管软镜碎石术后清石率的价值.方法 回顾性分析我院2024年1月至12月收治的163例肾结石患者的临床资料.术前根据CT平扫影像学资料均行3D成像和CT三维重建,分为3D成像组和CT三维重建组,两组分别行改良S.T.O.N.E评分,S.O.L.V.E评分以及SHA.LIN评分.根据数据绘制受试者工作特征曲线(ROC)并分析其预测结石清除的效能.对比两组在相同评分标准下的预测结石清除的曲线下面积(AUC).结果 结石清石率为82.8%,结石清除患者术前改良S.T.O.N.E评分,S.O.L.V.E评分以及SHA.LIN评分均低于结石残留患者(P<0.05),3D成像组在三种评分方式的AUC均高于CT三维重建组(P<0.05).结论 3D可视化技术在三种评分方式的预测效能均优于CT平扫三维重建,在预测输尿管软镜碎石术后清石率更具有优势.

Objective To compare the value of 3 D visualization technology versus CT non-contrast three-dimensional re-construction in the modified S.T.O.N.E.,S.O.L.V.E.,and SHA.LIN scoring systems for predicting stone-free rates(SFR)af-ter flexible ureteroscopic lithotripsy.Methods Clinical data from 163 patients with renal stones treated at our hospital between January and December 2024 were retrospectively analyzed.Preoperative 3D imaging and CT three-dimensional reconstruction were performed based on non-contrast CT scans,dividing patients into a 3D visualization group and a CT reconstruction group.Both groups were assessed using modified S.T.O.N.E.,S.O.L.V.E.,and SHA.LIN scores.Receiver operating characteristic(ROC)curves were plotted to analyze the predictive efficacy for stone clearance,and the area under the curve(AUC)was compared be-tween the two groups under identical scoring criteria.Results The overall stone-free rate(SFR)was 82.8%.Patients achieving stone-free status had significantly lower preoperative modified S.T.O.N.E.,S.O.L.V.E.,and SHA.LIN scores than those with residual stones(P<0.05).The AUC values for all three scoring methods were significantly higher in the 3D visualization group than those in the CT reconstruction group(P<0.05).Conclusion 3 D visualization technology outperforms CT three-dimensional reconstruction across all three scoring systems in predictive efficacy and demonstrates superior advantage in predicting SFR after flexible ureteroscopic lithotripsy.
