鼻咽癌放疗抵抗是导致中晚期患者治疗失败的主要原因,其核心机制在于肿瘤细胞通过异常激活 System Xc-/GSH/GPX4 抗氧化轴及 Nrf2 通路,从而逃逸铁死亡.黄连碱作为天然异喹啉类生物碱,可通过多靶点干预下调 SLC7A11 表达、耗竭谷胱甘肽、促进 GPX4 降解并抑制 Nrf2 核转位,有效瓦解铁死亡防御屏障.同时,黄连碱能与放疗诱导的活性氧产生协同致死效应,显著逆转放疗抵抗表型.然而,黄连碱存在水溶性差、生物利用度低等药代动力学缺陷,纳米递送系统(如脂质体、磁性纳米载体)为其临床转化提供了新策略.筛选 SLC7A11、GPX4 等生物标志物将有助于实现个体化精准用药.本文系统综述了黄连碱靶向铁死亡逆转鼻咽癌放疗抵抗的分子机制及临床转化前景,为开发新型放疗增敏药物提供理论依据.

Radiotherapy resistance in nasopharyngeal carcinoma(NPC)is a major cause of treatment failure in patients with locally advanced disease.The core mechanism involves the escaping of tumor cells from ferroptosis through aberrant activation of the System Xc-/GSH/GPX4 antioxidant axis and the Nrf2 pathway.Coptisine,a natural isoquinoline alkaloid,effectively dismantles the ferroptosis defense barrier through multi-target interventions,including downregulating SLC7A11 expression,depleting glutathione,promoting GPX4 degradation,and inhibiting Nrf2 nuclear translocation.Furthermore,coptisine exerts a synergistic lethal effect with radiotherapy-induced reactive oxygen species,significantly reversing the radioresistant phenotype.However,coptisine has pharmaco-kinetic limitations such as poor water solubility and low bioavailability.Nanodelivery systems(e.g.,liposomes,magnetic nanocarri-ers)offer new strategies for its clinical translation.Screening for biomarkers such as SLC7A11 and GPX4 will facilitate individualized precision medicine.This article systematically reviews the molecular mechanisms and clinical translation prospects of coptisine targe-ting ferroptosis to reverse radiotherapy resistance in nasopharyngeal carcinoma,providing a theoretical basis for developing novel radio-sensitizing agents.
