基于三维有限元分析不同滑脱程度的腰椎峡部裂原位融合术后相邻节段的生物力学变化OA
Analysis of biomechanical changes in adjacent segments after in situ fusion of lumbar spon-dylolisthesis with different degrees of slippage based on three-dimensional finite element
目的 探讨不同程度的腰椎峡部裂真性滑脱行原位融合术对相邻节段的生物力学影响.方法 基于三维有限元法建立不同程度滑脱的腰椎(L)4/骶椎(S)1 的腰椎滑脱原位融合与固定模型(复位固定模型、Ⅰ度滑脱原位固定模型、Ⅱ度滑脱原位固定模型、Ⅲ度滑脱原位固定模型).比较各模型的活动度及相邻 L4/5 节段椎间盘和关节突的应力分布.结果 与复位固定模型比较,Ⅰ度滑脱原位固定模型 L4/5 节段活动度与应力均无明显变化;Ⅱ度滑脱原位固定模型 L4/5 节段活动度相对增加且纤维环和关节面应力升高;Ⅲ度滑脱原位固定模型出现反常 L4/5 节段活动度和纤维环、关节面应力下降.结论 腰椎滑脱原位固定的生物力学影响具有分级依赖性:Ⅰ度滑脱在原位固定后,相邻节段退变(ASD)的风险较低;Ⅱ度滑脱则需注意 ASD 的发生风险;Ⅲ度滑脱应优先选择复位融合来恢复脊柱的稳定性.
Aim To explore the biomechanical effects of in situ fusion on adjacent segments in patients with lumbar spon-dylolytic true spondylolisthesis of varying degrees.Methods Lumbar spondylolisthesis in situ fusion and fixation models(reset fixed model,Ⅰ degree slip in situ fixation model,Ⅱ degree slip in situ fixation model,Ⅲ degree slip in situ fixation model)of lum-bar(L)4/sacral(S)1 with different degrees of slippage were established based on three-dimensional finite element.The activity of each model and the stress distribution of intervertebral discs and facet joints in adjacent L4/5 segments were compared.Results Compared with the reset fixed model,there was no significant change in the activity and stress of the L4/5 segment in Ⅰ degree slip in situ fixation model;The L4/5 segment of the Ⅱ degree slip in situ fixation model showed a relative increase in range of motion and in-creased stress on the annulus fibrosus and articular surface;The Ⅲ degree slip in situ fixation model showed abnormal L4/5 segment mobility and decreased stress on the fibrous ring and joint surface.Conclusion The biomechanical effects of in situ fixation for lumbar spondylolisthesis have a graded dependence:grade Ⅰ spondylolisthesis has a lower risk of adjacent segment degeneration(ASD)after in situ fixation;Attention should be paid to the risk of ASD occurrence in cases of grade Ⅱ slippage;Grade Ⅲ spondylo-listhesis should be prioritized to reduction and fusion to restore spinal stability.
