HIV感染者"检测不到=不传染"意义构建过程体验质性研究的Meta整合OA

Objective To systematically evaluate and synthesize the subjective experiences of HIV-positive individuals following their awareness of the scientific fact of"Undetectable equals Untransmittable"("U=U").Methods Qualitative studies were retrieved from databases including PubMed,Embase,SpringerLink,Cochrane Library,CNKI,Wanfang Data,VIP Database,Chinese Medical Library,and CBM,with a retrieval period spanning from database inception to January 23,2026.After two researchers independently screened the literature and extracted the data,the quality evaluation criteria for qualitative research of the Joanna Briggs Institute(JBI)in Australia were adopted,and the meta-integration was used to integrate the results.Results A total of 11 studies were included,and 22 findings were integrated into 3 results and 8 new categories:self-identity(the dynamic and complex psychological process of reconstructing and integrating identity cognition after learning about"U=U"),conditional freedom(the sense of freedom from the fact"U=U",entangled with deep-seated dependence and uncertainty regarding biomedical conditions and social recognition),and support needs(the dual gaps in medical communication and peer support during the integration of"U=U").Conclusion This study,for the first time,`through Meta-integration,revealed that the meaning construction of"U=U"for the infected individuals was a dynamic,con-tradictory and support-dependent process,going beyond the simple description of psychological benefits or obstacles,and providing an integrated framework for psychological and social support of HIV-positive individuals."U=U"profoundly affects the psychological and social adaptation and identity reconstruction of HIV-positive individuals.Medical staff should pay attention to the dynamic changes of their experiences and provide phased and individualized support to promote their positive identity integration and life reconstruction.
