目的:探究下肢康复机器人辅助训练联合等速肌力训练在改善老年脑卒中偏瘫患者神经功能与运动能力方面的临床效果.方法:回顾性分析 2024 年 1 月—2025 年 1 月本院收治的 118 例老年脑卒中偏瘫患者的临床资料,根据康复方案分为对照组(n=50,采用常规康复训练)和观察组(n=68,采用常规康复+下肢康复机器人辅助联合等速肌力训练),两组康复训练的疗程均为 12 周.比较两组治疗前和治疗后的下肢运动功能[Fugl-Meyer 下肢运动功能评定量表(FMA-LE)评分]、平衡功能及步态参数[Berg 平衡量表(BBS)评分、起立-行走计时测试(TUGT)时间、10 米步行测试(10MWT)时间]、等速肌力测试、日常生活活动能力与神经功能[改良Barthel指数(MBI)以及美国国立卫生研究院卒中量表(NIHSS)评分],以及不良反应与训练依从性.结果:治疗后,观察组 FMA-LE 各维度评分及总分、BBS 评分、步速、步长、步频及步长对称性指数均显著优于对照组(P<0.05);TUGT 时间和 10MWT 时间显著短于对照组(P<0.05);膝关节和髋关节各肌群峰力矩/体重比值或总功显著大于对照组(P<0.05);MBI 评分显著高于对照组,NIHSS 评分显著低于对照组(P<0.05).两组患者的不良反应发生率及依从性相较,差异均无统计学意义(P>0.05).结论:下肢康复机器人联合等速肌力训练可全方位改善老年脑卒中偏瘫患者的运动功能、平衡能力、步态参数、肌力水平及日常生活活动能力,其综合康复效果显著优于单纯常规康复训练,且安全性良好,值得临床推广应用.

Objective:To investigate the clinical effect of lower limb rehabilitation robot-assisted training combined with isokinetic muscle strength training on improving neurological function and motor ability in elderly stroke patients with hemiplegia.Methods:A retrospective analysis was conducted on the clinical data of 118 elderly stroke patients with hemiplegia who were treated at our hospital from January 2024 to January 2025.According to the rehabilitation program,they were divided into the the control group(n=50,receiving only conventional rehabilitation training)and the observation group(n=68,receiving lower limb rehabilitation robot-assisted training combined with isokinetic muscle strength training in addition to conventional rehabilitation).Both groups underwent a 12-week rehabilitation program.The lower limb motor function[Fugl-Meyer assessment of lower extremity(FMA-LE)score],balance function and gait parameters[Berg balance scale(BBS)score,timed up-and-go test(TUGT)and 10-meter walk test(10MWT)],isokinetic muscle strength test,activities of daily living(ADLs)and neurological function[modified Barthel index(MBI)and National Institutes of Health stroke scale(NIHSS)score],adverse reactions,and training compliance were compared between the two groups before and after treatment.Results:After treatment,the observation group showed significantly higher scores than the control group in all dimensions and total scores of FMA-LE,BBS score,walking speed,step length,step frequency,and step length symmetry index(P<0.05).TUGT time and 10MWT time were significantly shorter in the observation group than those in the control group(P<0.05).The peak torque/body weight ratio or total work of each muscle group of the knee and hip joints was significantly greater in the observation group than that in the control group(P<0.05).The MBI score was significantly higher and the NIHSS score was significantly lower in the observation group than those in the control group(P<0.05).There were no significant differences in the incidence of adverse reactions and compliance between the two groups(P>0.05).Conclusion:Lower limb rehabilitation robot-assisted training combined with isokinetic muscle strength training can comprehensively improve motor function,balance ability,gait parameters,muscle strength level and ADLs in elderly stroke patients with hemiplegia.With good safety,its comprehensive rehabilitation effect is significantly better than that of conventional rehabilitation training alone,and it is worthy of clinical promotion and application.
