保留耻骨膀胱复合体在机器人辅助腹腔镜根治性前列腺切除术中的应用(附手术视频)OA

Objective:To investigate the clinical effects of pubovesical complex preserviation in robot-assisted laparoscopic radical prostatectomy(RARP).Methods:86 prostate cancer patients who underwent RARP at Xi'an Gaoxin Hospital from February 2020 to February 2025 were enrolled in this study.The patients were divided into the control group(n=43,without preservation of the pubovesical complex)and the study group(n=43,with preservation of the pubovesical complex)using a random number table.Perioperative outcomes,urinary continence,quality of life,and emotional status were compared between the two groups.Results:There was no statistically significant difference in operative time between the two groups(P>0.05).The incidence of urinary incontinence at 24 h,one month,three months,and six months after catheter removal was significantly lower in the study group than that in the control group(P<0.05).After intervention,scores on all items of the International Consultation on Incontinence questionnaire-short form(ICI-Q-SF)decreased in the two groups,with the study group scoring lower than the control group(P<0.05).After intervention,scores on the incontinence quality of life questionnaire(I-QOL)increased in the two groups,with the study group scoring higher than the control group(P<0.05).One and three months after the intervention,scores on the self-rating anxiety scale(SAS)and the self-rating depression scale(SDS)decreased in the two groups,with the study group scoring lower than the control group(P<0.05).Conclusion:Preservation of the pubovesical complex in RARP can improve postoperative recovery of patients,effectively enhance urinary continence and quality of life,reduce the incidence of urinary incontinence,and ameliorate emotional status.
