基于倾向评分匹配比较多孔机器人辅助与单孔电视辅助胸腔镜治疗非小细胞肺癌的近期疗效OA
A comparison of short-term efficacy between multiport robot-assisted and uniport video-assisted thoracoscopic surgery for non-small cell lung cancer based on propensity score matching
目的:比较多孔机器人辅助胸腔镜手术(M-RATS)与单孔电视辅助胸腔镜手术(U-VATS)在非小细胞肺癌(NSCLC)患者中的近期疗效,探讨其临床应用价值.方法:回顾性分析 2023 年 5 月—2024 年 8 月于新疆医科大学附属肿瘤医院肺外二科接受 U-VATS及M-RATS的NSCLC患者临床资料,进行倾向评分匹配(PSM)后比较两组患者围手术期指标.结果:共纳入 378 例患者,其中 M-RATS 组 157 例,U-VATS 组 221 例,PSM 后各纳入 124 例.两组患者均顺利完成手术,无围手术期死亡病例.PSM 后,与 U-VATS 组相比,M-RATS 组手术时间更长,胸管留置时间更短,清扫淋巴结站数及个数更多,住院总花费及术后并发症总发生率更高(P<0.05).结论:接受 M-RATS 与 U-VATS 患者的围手术期结局相当,但前者可实现更彻底的淋巴结清扫,有助于精准评估淋巴结状态及肿瘤分期,从而优化NSCLC患者的肿瘤学治疗效果.
Objective:To compare the short-term efficacy of multiport robot-assisted thoracoscopic surgery(M-RATS)and uniport video-assisted thoracoscopic surgery(U-VATS)in patients with non-small cell lung cancer(NSCLC)and evaluate their clinical value.Methods:A retrospective analysis was conducted on the clinical data of NSCLC patients who underwent U-VATS or M-RATS in the Department of Pulmonary Surgery Ⅱ,Affiliated Tumor Hospital of Xinjiang Medical University from May 2023 to August 2024.Perioperative indicators of the two groups were compared after propensity score matching(PSM).Results:A total of 378 patients were included,with 157 in the M-RATS group and 221 in the U-VATS group.After PSM,124 patients were included in each group.All surgeries were successfully completed without perioperative mortality.After PSM,the M-RATS group had significantly longer operative time,shorter duration of chest tube drainage,greater number of dissected lymph nodes and nodal stations compared to the U-VATS group(P<0.05).Additionally,total hospitalization costs and the overall incidence of postoperative complications were significantly higher in the M-RATS group than the U-VATS group(P<0.05).Conclusion:M-RATS and U-VATS have comparable perioperative outcomes in NSCLC patients.The former can achieve more thorough lymph node dissection,which has superior advantages in precisely assessing lymph node status and tumor staging,thereby optimizing oncological outcomes in NSCLC patients.
