四维职业认同感量表在护士群体中的修订及信效度检验OA

目的:在现有职业认同四维结构模型(职业意志、职业意愿、职业价值、职业效能)基础上进行量表修订与验证,为精准评估护理人员职业认同水平提供标准化工具.方法:通过文献回顾与半结构化访谈构建初始条目池,经两轮德尔菲专家咨询确立初始量表.采用多阶段抽样策略,预测试阶段有效调查广东省某三甲医院护士 227名,通过项目分析(临界比值法、题总相关、CITC)筛选条目;正式调查阶段在全国6大行政区的21家医院中有效调查421名护士.运用探索性因子分析(EFA)和验证性因子分析(CFA)等方法系统检验量表的心理测量学特性.结果:最终护士四维职业认同感量表保留12个条目,涵盖四维度.EFA显示,4个因子累计解释方差79.731%;CFA 拟合指标优良(x2/df=1.186,RMSEA=0.021,CFI=0.997,TLI=0.996,IFI=0.997).效标效度显著(与刘玲版r=0.798,赵红版r=0.826;P<0.001)职业认同量表具有高相关性.总量表Cronbach's α系数达0.908,各维度Cronbach's α系数介于0.847~0.874.结论:修订的12条目护士四维职业认同量表信效度优良,兼具测量精确性与临床实用性,能适用于护士群体职业认同的快速筛查,为未来研究提供工具支持.

Objective:To revise and validate a scale based on the existing four-dimensional structure model of pro-fessional identity(comprising professional will,professional intention,professional value,and professional efficacy),providing a standardized tool for accurately assessing the professional identity level of nursing staff.Methods:An ini-tial item pool was constructed through literature review and semi-structured interviews,and the initial scale was es-tablished after two rounds of Delphi expert consultation.Using a multi-stage sampling strategy,227 nurses from a tertiary hospital in Guangdong Province were effectively surveyed during the pre-test phase.Items were screened through item analysis(critical ratio method,item-total correlation,CITC).In the formal survey phase,421 nurses from 21 hospitals across six major administrative regions in China were effectively surveyed.Psychometric properties of the scale were systematically tested using exploratory factor analysis(EFA)and confirmatory factor analysis(CFA).Results:The final version of the Four-Dimensional Nurse Professional Identity Scale retained 12 items,cov-ering four dimensions.EFA showed that the four factors cumulatively explained 79.731%of the variance.CFA dem-onstrated excellent model fit(x2/df=1.186,RMSEA=0.021,CFI=0.997,TLI=0.996,IFI=0.997).Criterion validi-ty was significant,with high correlations to the Liu Ling version(r=0.798)and the Zhao Hong version(r=0.826)(P<0.001).The total scale's Cronbach's α coefficient was 0.908,and the Cronbach's α coefficients for each dimension ranged from 0.847 to 0.874.Conclusion:The revised 12-item Four-Dimensional Nurse Professional Identity Scale exhibits excellent reliability and validity,combining measurement precision with clinical practicality.It is suitable for rapid screening of professional identity among nurses and provides tool support for future research.
