目的 探讨实时动态血糖监测(CGM)在糖尿病肾病(DKD)血液透析患者优化治疗指导中的应用价值.方法 选取2019年11月至2023年12月深圳市龙华区人民医院收治的60例DKD血液透析患者作为研究对象,根据随机数字表法分成观察组和对照组,每组各30例.观察组行实时CGM,对照组行手指血糖监测仪监测.比较两组的血糖控制情况、肾功能指标、自我管理情况,统计监测期间并发症发生情况.结果 观察组日均高血糖持续时间短于对照组,日均血糖波动范围小于对照组,差异有统计学意义(P<0.05).监测14 d后空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)、24h尿蛋白定量(24hUTP)、血尿素氮(BUN)、血清肌酐(SCr)水平低于对照组,差异有统计学意义(P<0.05).两组监测14 d后糖尿病自我管理行为量表(SDSCA)评分高于监测前,中文版低血糖恐惧感调查-忧虑量表(CHFS Ⅱ-WS)评分低于监测前,且观察组SDSCA评分高于对照组,CHFS Ⅱ-WS评分低于对照组,差异有统计学意义(P<0.05).两组并发症总发生率比较,差异无统计学意义(P>0.05).结论 实时CGM技术应用在DKD血液透析患者中可有效提高血糖控制效果,减少患者血糖波动,缩短高血糖持续时间,保护肾功能,增强患者自我管理能力及缓解低血糖恐惧.

Objective To explore the application value of real-time continuous glucose monitoring(CGM)in the optimiza-tion of treatment guidance for patients with diabetic kidney disease(DKD)undergoing hemodialysis.Methods A total of 60 patients with DKD on hemodialysis admitted to Longhua District People's Hospital from November 2019 to December 2023 were selected as the research objects.According to the random number table method,they were divided into the observation group and the control group,with 30 cases in each group.The observation group received real-time CGM,while the control group received finger blood glucose monitoring.The blood glucose control status,renal function indicators,self-management situation of the two groups were compared,and the occurrence of complications during the monitoring period was statistically analyzed.Results The average duration of hyperglycemia in the observation group was shorter than that in the control group,and the average range of blood glucose fluctuations was smaller than that in the control group,the differences were statistically significant(P<0.05).After 14 days of monitoring,the levels of fasting plasma glucose(FPG),2 hour postprandi-al plasma glucose(2 h PG),glycated haemoglobin(HbAlc),24 hour urinary total protein(24 h UTP),blood urea nitrogen(BUN),and serum creatinine(SCr)were lower in the observation group than in the control group,and the differences were statistically significant(P<0.05).After 14 days of monitoring,the scores of the summary of diabetes self-care activities(SDSCA)in both groups was higher than those before monitoring,while the scores of the Chinese version hypoglycemia fear surveyll-worry scale(CHFS Ⅱ-WS)were lower than those before monitoring,the SDSCA score in the observation group was higher than that in the control group,and the CHFS Ⅱ-WS score was lower than that in the control group,and the differ-ences were statistically significant(P<0.05).There was no statistically significant difference in the total incidence of com-plications between the two groups(P>0.05).Conclusion The application of real-time CGM technology in DKD hemodialy-sis patients can effectively improve the effect of blood glucose control,reduce blood glucose fluctuations in patients,shorten the duration of hyperglycemia,protect renal function,enhance patients' self-management ability and alleviate the fear of hy-poglycemia.
