育肾消癥方对子宫腺肌病患者激素水平的影响OA
The Effect of Self-Prescribed Kidney-Tonifying and Mass-Dissolving Formula on Hormone Levels in Patients with Adenomyosis
目的:探讨自拟育肾消癥方对子宫腺肌病患者激素水平的影响.方法:选取周口市中医院2022 年3 月至2024 年6 月收治的94 例子宫腺肌病患者为研究对象,按照随机数字表法分为对照组和观察组,每组各 47 例.对照组给予屈螺酮炔雌醇片治疗,观察组在对照组治疗的基础上联合自拟育肾消癥方治疗.比较两组患者的临床疗效、不良反应发生率、复发情况及治疗前后子宫内膜厚度、子宫体积、月经量与痛经程度评分[月经失血图评分法(pictorial blood loss assessment chart,PBAC)、COX痛经症状评分量表(cox menstrual symptom scale,CMSS)]、疼痛与应激相关指标[前列腺素 F2α(prostaglandin F2 alpha,PGF2α)、P 物质(substance P,SP)、β-内啡肽(β-endorphin,β-EP)]、激素水平[卵泡刺激素(follicle-stimulating hormone,FSH)、促黄体生成素(luteinizing hormone,LH)、雌二醇(estradiol,E2)]变化情况.结果:观察组有效率为91.49%,高于对照组的76.60%,差异具有统计学意义(P<0.05).两组患者治疗后 CMSS 评分、PBAC 评分、子宫体积、子宫内膜厚度低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05).两组患者治疗后 PGF2α、SP 低于本组治疗前,β-EP 水平高于本组治疗前,且治疗后组间比较,差异具有统计学意义(P<0.05).两组患者治疗后 E2 水平高于本组治疗前,LH、FSH水平低于本组治疗前,且治疗后组间比较,差异具有统计学意义(P<0.05).两组患者不良反应发生率及复发情况比较,差异无统计学意义(P>0.05).结论:自拟育肾消癥方治疗子宫腺肌病,可改善患者子宫大小及月经量,缓解痛经程度,其作用机制可能与调节雌激素水平、调控疼痛相关因子表达有关.
Objective:To investigate the effect of Self-Prescribed Kidney-Tonifying and Mass-Dissolving Formula on hormone levels in pa-tients with adenomyosis.Methods:A total of 94 patients with adenomyosis admitted to Zhoukou Hospital of Traditional Chinese Medicine from March 2022 to June 2024 were selected as the study subjects,and divided into the control group and the observation group using the coin toss method,with 47 cases in each group.The control group was treated with Drospirenone and Ethinylestradiol Tablets,while the observation group received Self-Prescribed Kidney-Tonifying and Mass-Dissolving Formula in addition to the control group's treat-ment.The clinical efficacy,adverse reactions,recurrence rate,changes in endometrial thickness,uterine volume,menstrual flow and dys-menorrhea severity score menstrual blood loss[including pictorial blood loss assessment chart(PBAC),cox menstrual symptom scale(CMSS)],pain and stress-related indicators[including prostaglandin F2 alpha(PGF2α),substance P(SP),β-endorphin(β-EP)],and estrogen levels[including follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2)]before and after treat-ment were compared between the two groups.Results:The effective rate in the observation group was 91.49%,which was higher than the 76.60%in the control group,and the difference was statistically significant(P<0.05).After treatment,the CMSS and PBAC scores,uterine volume,and endometrial thickness in the two groups were all lower than those before treatment within each group,and these parameters in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).PGF2α and SP levels in the two groups were all lower than those before treatment within each group,while β-EP levels were higher,with statistically significant differences(P<0.05).E2 levels in the two groups were all higher than those before treatment with-in each group,while LH and FSH levels were lower,with statistically significant differences(P<0.05).However,there was no statisti-cally significant difference in the incidence of adverse reactions or the recurrence rate between the two groups(P>0.05).Conclusion:Self-Prescribed Kidney-Tonifying and Mass-Dissolving Formula in the treatment of adenomyosis can improve uterine size and reduce menstrual blood loss,alleviate the severity of dysmenorrhea.Its mechanism of action may be related to regulating estrogen levels and modulating the expression of pain-related factors.
