目的 探讨中性粒细胞-淋巴细胞比率(NLR)、淋巴细胞-单核细胞比率(LMR)及血浆血小板微粒(PMP)水平与急性脑梗死(ACI)患者接受静脉溶栓(IVT)治疗后功能结局的相关性.方法 采用回顾性研究方法,选取2022-05-2024-12在新疆医科大学第二附属医院接受IVT治疗的70例ACI患者为研究对象.根据预后结果分为良好组(n=44)和不良组(n=26).比较2组患者基线资料、实验室指标(NLR、LMR及血浆PMP水平),分析接受IVT治疗ACI患者的NLR、LMR及血浆PMP水平与预后结局的相关性,采用Logistic回归分析患者预后不良的独立危险因素.结果 不良组NIHSS评分显著高于良好组(P<0.05).不良组NLR、血浆PMP水平均高于良好组,LMR水平低于良好组(P<0.05).相关性分析显示,NLR、血浆PMP水平与基线NIHSS、mRS评分均呈显著正相关(P<0.05),LMR水平与其均呈负相关(P<0.05).Logistic分析显示,NLR升高、LMR降低及血浆PMP升高是患者预后不良的独立危险因素(P<0.05).结论 NLR、LMR及血浆PMP水平与ACI患者IVT后功能结局密切相关,可作为预测预后的重要生物学指标.

Objective To investigate the associations of neutrophil-lymphocyte ratio(NLR),lymphocyte-monocyte ratio(LMR),and plasma platelet microparticle(PMP)levels with functional outcomes in acute cerebral infarction(ACI)patients by intravenous thrombolysis(IVT).Methods A retrospective selection was made of 70 patients with ACI who underwent IVT treatment at the Second Affiliated Hospital of Xinjiang Medical University from May 2022 to December 2024 as the research subjects.According to the prognosis results,they were divided into the good group(n=44)and the poor group(n=26).The baseline data and laboratory indicators(NLR,LMR and plasma PMP levels)of the two groups were compared.The correlation between the levels of NLR,LMR and plasma PMP in ACI patients by IVT treatment and the prognosis outcome was analyzed.Independent predictors of poor prognosis were identified using multivariate Logistic regression.Results AThe baseline NIHSS score was significantly higher in patients with poor prognosis compared to those with favorable outcome(P<0.05).The levels of NLR and plasma PMP in the poor group were both higher than those in the good group.However,the LMR level was lower than that of the good group(P<0.05).Correlation analysis showed that the NLR level and plasma PMP level were significantly positively correlated with the baseline NIHSS score and mRS Score(P<0.05).The LMR level was negatively correlated with both scores(P<0.05).Logistic analysis showed that elevated NLR,decreased LMR,and elevated plasma PMP were independent risk factors(P<0.05).Conclusion NLR,LMR and plasma PMP levels are closely related to the functional outcomes of ACI patients after IVT and can be used as important biological indicators for predicting the prognosis.
