合并亚临床甲状腺功能减退的脑梗死临床特点及影响因素OA
Clinical characteristics and influencing factors of cerebral infarction complicated with subclinical hypothyroidism
目的 分析合并亚临床甲状腺功能减退(SCH)的动脉粥样硬化性脑梗死(ASCI)的临床特点及影响因素.方法 回顾性收集2020-01-01-2022-08-01在首都医科大学附属北京同仁医院神经内科住院接受诊治的ASCI患者的临床资料.采用倾向性评分匹配法分析合并SCH的ASCI患者的临床特点,采用单因素和多因素Logistic回归分析其影响因素.结果 SCH组合并高血压(74.36%比93.91%,P<0.001)、高脂血症(15.38%比85.28%,P<0.001)、高同型半胱氨酸血症(5.13%比20.30%,P<0.001)比例显著低于无SCH组.C反应蛋白(OR=0.735,95%CI:0.572~0.924,P=0.011)、中性粒细胞(OR=1.964,95%CI:1.263~3.210,P=0.004)、高血压(OR=0.178,95%CI:0.042~0.722,P=0.016)、高脂血症(OR=0.021,95%CI:0.006~0.062,P<0.001)是合并SCH的ASCI的影响因素.结论 C反应蛋白、中性粒细胞、高血压、高脂血症可能是合并SCH的ASCI的影响因素.
Objective To analyze the clinical characteristics of arteriosclerotic cerebral infarction(ASCI)complicated with subclinical hypothyroidism(SCH),and explore its influencing factors.Methods This retrospective study collected clinical data from ASCI patients admitted to the Department of Neurology,Beijing Tongren Hospital,Capital Medical University between January 1,2020 and August 1,2022.The propensity score matching method was used to analyze the clinical characteristics of ASCI patients complicated with SCH,and univariate and multivariate Logistic regression were applied to explore its influencing factors.Results The SCH group had significantly lower rates of hypertension(74.36%vs 93.91%,P<0.001),hyperlipidemia(15.38%vs 85.28%,P<0.001),and hyperhomocysteinemia(5.13%vs 20.30%,P<0.001)compared to the non-SCH group.C-reactive protein(OR=0.735,95%CI:0.572-0.924,P=0.011),neutrophils(OR=1.964,95%CI:1.263-3.210,P=0.004),hypertension(OR=0.178,95%CI:0.042-0.722,P=0.016),and hyperlipidemia(OR=0.021,95%CI:0.006-0.062,P<0.001)as influencing factors for ASCI complicated with SCH.Conclusion C-reactive protein,neutrophils,hypertension,and hyperlipidemia may be influencing factors for ASCI complicated with SCH.
