核因子E2 相关因子2(nuclear factor erythroid 2-related factor 2,Nrf2)是调控细胞氧化应激的重要因子,在缺血性脑卒中病理机制中具有调节细胞程序性死亡、抗氧化应激、抗神经炎症、维持血脑屏障稳态、促进血管生成、促进神经再生等作用.萜类、苷类、生物碱类等中药单体,补阳还五汤、黄芪桂枝五物汤、化浊解毒活血通络方、益脑复健方等中药复方均可通过调控Nrf2 相关信号通路治疗缺血性脑卒中.现有研究存在如下不足之处:中药促进血管和神经再生的研究较少;中药单体作用机制研究居多,复方研究薄弱;药物安全剂量和最大疗效剂量研究仍需深入;部分实验设计有待完善;Nrf2 多途径、多通路的协同调节机制有待深入研究.针对此,未来应加强中药复方研究,探索 Nrf2 多通路协同调控机制,以期为缺血性脑卒中的中医药治疗提供新思路与新方法.

Nuclear factor erythroid 2-related factor 2(Nrf2)is an important factor in regulating cellular oxidative stress.In the patho-logical mechanism of ischemic stroke,it plays roles in regulating programmed cell death,antioxidative stress,anti-neuroinflammation,maintaining blood-brain barrier homeostasis,promoting angiogenesis,and facilitating neural regeneration.Monomeric components of traditional Chinese medicine(TCM)such as terpenoids,glycosides,and alkaloids,as well as TCM formulas including Buyang Huanwu Decoction,Huangqi Guizhi Wuwu Decoction,Huazhuo Jiedu Huoxue Tongluo Formula,and Yinao Fuyan Formula,all can treat ischemic stroke by regulating Nrf2-related signaling pathways.Existing studies have the following shortcomings:there is limited research on the promotion of vascular and neural regeneration by TCM;studies mainly focus on monomer mechanisms,while research on formulas is weak;studies on safe dosage and maximum therapeutic dosage of drugs still need deepening;some experimental designs need improve-ment;and the synergistic regulatory mechanisms of Nrf2 through multiple pathways remain to be further studied.In response,future re-search should strengthen studies on TCM formulas and explore the synergistic regulation mechanisms of Nrf2 across multiple pathways,aiming to provide new ideas and methods for the TCM treatment of ischemic stroke.
