基于线粒体稳态从"一气周流"论治心肌缺血再灌注损伤OA
Treatment of Myocardial Ischemia-Reperfusion Injury Based on Mitochondrial Homeostasis from Perspective of"Yi Qi Zhou Liu"
心肌缺血再灌注损伤(myocardial ischemic reperfusion injury,MIRI)的基本病机是中气不运,水寒木郁火衰,痰瘀凌火,气虚血瘀,致胸阳不振,心脉痹阻,心失荣养.诸多药物可通过调节线粒体稳态,减轻 MIRI 过程中一系列的病理反应,改善心肌缺血/再灌注导致的心肌损伤.临证基于"一气周流"理论,以培土制水,通阳化痰;扶中益火,行气化瘀;益气升提,理气开郁为治疗原则,通过持中土以运四傍,培调中土,复其枢轴升降之权,调顾四维,使脏腑气血阴阳流转顺畅,使线粒体生物发生、自噬及分裂融合平衡协调,促进线粒体稳态恢复,进而减轻 MIRI.
The fundamental pathogenesis of myocardial ischemic reperfusion injury(MIRI)is the dysfunction of central Qi transporta-tion,Cold in Water,constrained in Wood and declining in Fire,Phlegm and blood stasis overwhelming Fire,Qi deficiency with blood sta-sis,leading to insufficient Heart Yang,obstruction of the Heart vessels,and loss of nourishment to the Heart.Many drugs can mitigate a series of pathological reactions during MIRI by regulating mitochondrial homeostasis,thereby improving myocardial damage caused by ischemia/reperfusion.Clinically,based on the theory of"Yi Qi Zhou Liu",the treatment principle is to strengthen the Earth to control Water,unblock Yang to resolve Phlegm;support the center to enhance Fire,promote Qi circulation to remove stasis;benefit and lift Qi,regulate Qi to relieve stagnation.By maintaining the central Earth to operate the whole body,nurturing and regulating central Earth to re-store the authority of its pivot in Qi ascending and descending,and harmonizing each part of the body,the flow of Qi,blood as well as Yin and Yang in the Zang and Fu become smooth,leading to coordinated regulation of mitochondrial biogenesis,autophagy,and fission/fusion balance,promoting the restoration of mitochondrial homeostasis and thereby alleviating MIRI.
