黎世明从肺脾论治小儿鼻鼽的临床经验OA
Li Shiming's Clinical Experience in Treating Pediatric Sniveling Nose from the Perspective of Lung and Spleen
鼻鼽是儿童常见的疾病之一,现代医学称之为变应性鼻炎.黎世明教授认为,变应性鼻炎患儿的脾虚体质较明显.病机多以肺卫不固、脾虚失运为核心.针对其肺脾两虚的病机,治疗上提出宣肺通窍以治标、健脾益气以治本、肺脾同调以固效的原则.健脾益气、固表通窍的常用药物包括黄芪、五指毛桃、太子参、辛夷、炒苍耳子、五味子、白术等.临证灵活加减,强调随证调整攻补比例,从而实现标本兼顾、肺脾同调.黎世明教授从肺脾论治儿童变应性鼻炎的经验,可为儿童变应性鼻炎的中医临床诊治提供参考.
Sniveling nose,known as allergic rhinitis in modern medicine,is one of the common pediatric disorders.Professor Li Shiming posits that children with allergic rhinitis frequently present with a pronounced spleen-deficient constitution.The core pathogenesis is predominantly characterized by insecurity of defensive qi of the lung and failure in transportation due to spleen deficiency.In response to the dual deficiency of the lung and spleen,Professor Li recommends the therapeutic principles of diffusing the lung and unblocking the orifices to address the manifestations,strengthening the spleen and replenishing qi to treat the root cause,and simultaneously regulating both the lung and spleen to consolidate long-term efficacy.There are Astragali Radix,Fici Simplicissimae Radix,Pseudostellariae Radix,Magnoliae Flos,fried Xanthii Fructus,Schisandrae Chinensis Fructus,and Atractylodis Macrocephalae Rhizoma frequently used for strengthening the spleen,replenishing qi,securing the exterior,and unblocking the orifices.In clinical practice,modifications are performed flexibly,with careful adjustment of the ratio of purging herbs to tonifying herbs according to syndrome differentiation,thereby achieving concurrent management of both manifestations and root cause and simultaneous regulation of the lung and spleen.Professor Li's experience in treating pediatric allergic rhinitis from the perspective of lung and spleen offers valuable insights for its diagnosis and management with traditional Chinese medicine.
