基于"督脉通阳"探讨针灸治疗脊髓损伤的小胶质细胞调控机制OA
Microglial Regulation by Acupuncture-Moxibustion Therapy in Spinal Cord Injury:A Mechanistic Exploration from"Governor Vessel-Mediated Yang Activation"
脊髓损伤(SCI)是一种以神经功能障碍为核心表现的重大创伤性疾病,中医"督脉通阳"理论为其治疗提供了独特的视角.本研究创新性地构建了"督脉通阳-小胶质细胞极化"理论模型,提出针灸通过调控"小胶质细胞M1/M2表型转化"改善SCI的机制,并解析针灸双向调节的优势及不同疗法的效应差异.阐释针灸可有效抑制促炎性M1型极化、促进修复性M2型极化,重塑神经炎症微环境,打破"促炎-损伤-修复抑制"恶性循环,为针灸治疗SCI提供了新的理论支撑.
Spinal cord injury(SCI)is a major traumatic disease characterized by neurological dysfunction.The traditional Chinese medicine theory of"Governor vessel-mediated yang activation"provides a unique perspective for its treatment.This study innovatively constructs a theoretical model of"Governor vessel-mediated yang activation",proposing that acupuncture-moxibustion therapy improves SCI by regulating microglial M1/M2 phenotype transformation.It further analyzes the advantages of the bidirectional regulatory effect of acupuncture-moxibustion therapy and the differences in efficacy among various acupuncture-moxibustion therapy modalities.The findings elucidate that acupuncture-moxibustion therapy can effectively inhibit pro-inflammatory M1 polarization,promote reparative M2 polarization,reshape the neuroinflammatory microenvironment,and break the vicious cycle of"pro-inflammation-injury-repair inhibition",thereby providing new theoretical support for the treatment of SCI with acupuncture-moxibustion therapy.
