"H2H"营养管理模式对胆囊癌根治术患者营养状况及生活质量的影响OA

Summary This study aimed to assess the effect of the"H2H"(hospital-to-home)nutritional management model on the nutritional status,quality of life,anxiety,depression,and clinical outcomes in patients undergoing radical resection for gallbladder cancer(GBC).Eighty patients scheduled for radical GBC resection were randomly assigned to either a control group,which received routine nutritional management,or an observation group,which received the"H2H"model in addition to routine care.The"H2H"model comprised multidisciplinary collaboration,individualized nutritional support during hospitalization,and continuous post-discharge home-based management facilitated by WeChat,food diaries,and regular follow-up consultations.Nutritional indicators(albumin,total protein,hemoglobin),quality of life(SF-36),anxiety and depression(HADS),complications,time of hospital stay,and hospitalization costs were compared between the two groups at admission,at discharge,and at 1-month post-discharge.Results showed that,at 1-month post-discharge,the observation group demonstrated significantly higher levels of albumin,total protein,and hemoglobin(all P<0.05).Furthermore,both at discharge and at 1-month post-discharge,the observation group exhibited significantly higher SF-36 scores and significantly lower HADS scores(both P<0.001).The observation group also experienced lower rates of infection and liver dysfunction,shorter time of hospital stay,and reduced hospitalization costs(all P<0.05).No significant difference was observed in the incidence rate of bile leakage or hemorrhage.In conclusion,for patients undergoing radical resection of GBC,the"H2H"nutritional management model effectively improves nutritional status,enhances quality of life,alleviates anxiety and depression,and reduces postoperative complications,time of hospital stay,and associated costs.
