数字医疗在心脏康复中的应用中国专家共识OA
Chinese Expert Consensus on Application of Digital Healthcare in Cardiac Rehabilitation
心脏康复(CR)是心血管疾病管理的重要措施,可改善运动耐量、生活质量;降低再住院率与病死率.但受限于医疗资源分布不均、患者依从性低、随访管理困难等因素,心脏康复的实施率仍偏低.数字医疗有助于解决传统心脏康复在可行性、连续性及个性化方面的难题.本共识主要涵盖远程心脏康复在运动处方(冠状动脉粥样硬化性心脏病、心力衰竭、心脏瓣膜病)、营养处方(饮食远程指导、基于聊天机器人的饮食监测)、心理处方(心理筛查评估、心理康复实施与干预及监督与反馈)中的应用,并提出相应专家共识推荐;同时总结归纳了数字化冠状动脉CT血流储备分数、冠脉微循环阻力指数在心脏康复中的应用及专家推荐意见.本共识有助于推进基于数字技术的远程心脏康复技术的科学、规范化应用.
Cardiac rehabilitation(CR)is a crucial component of cardiovascular disease management.It effectively enhances exercise tolerance and quality of life,while reducing the rates of readmission and mortality.However,its implementation remains suboptimal due to challenges such as uneven distribution of medical resources,low patient adherence,and difficulties in follow-up management.Digital healthcare offers potential solutions to overcome the limitations of traditional cardiac rehabilitation regarding fea-sibility,continuity,and personalization.This consensus primarily addresses the application of remote cardiac rehabilitation,including exercise prescriptions for coronary artery disease,heart failure,and valvular heart disease;nutritional prescriptions with remote dietary guidance and chatbot-based dietary monitoring;and psychological prescriptions covering screening,intervention,and feed-back.It provides corresponding expert consensus recommendations and summarizes the clinical utility of digital coronary CT-de-rived fractional flow reserve(CT-FFR)and index of microcirculatory resistance(IMR)in CR,along with expert-endorsed opinions.This consensus aims to promote the scientific and standardized application of digital technology-based remote cardiac rehabilitation.
