Application of Transcutaneous Auricular Vagus Nerve Stimulation in Motor Rehabilitation after Stroke
经皮耳迷走神经刺激(taVNS)与侵入式迷走神经刺激(iVNS)能激活同样的神经通路,具有低成本、安全和耐受性良好等优点,可作为iVNS的替代神经调控技术,在脑卒中后运动功能康复中具有良好的应用前景.但taVNS具体应用参数和调控效果缺乏统一性,且潜在作用机制尚不明确.本综述就taVNS在脑卒中后运动功能康复中的应用(taVNS非同步联合上肢训练治疗、taVNS同步联合上肢训练治疗和taVNS单独治疗)、作用机制(抗炎和神经保护,增强血脑屏障完整性,增强皮质兴奋性和神经可塑性)以及刺激参数、安全性和刺激效果进行分析和讨论.目前缺乏高质量临床研究,大部分taVNS研究仅评估肢体功能评分,未在神经生理层面分析其作用机制;taVNS刺激参数(如刺激强度、刺激频率、刺激时长和刺激时机等)尚未形成统一共识;taVNS右耳的安全性需要进一步验证;少量研究发现taVNS左耳产生了偏侧化效应.未来还需要更多大样本量的高质量研究来确定最佳刺激参数和探究作用机制,为临床上制定最佳taVNS治疗方案提供参考.
Transcutaneous auricular vagus nerve stimulation(taVNS)activates neural pathways akin to invasive vagus nerve stimulation(iVNS).With the advantages of low cost,safety,and high tolerability,taVNS serves as an alternative neuromodulatory technique to iVNS and presents promising prospects for motor function rehabilitation after stroke.However,there is a lack of con-sensus on specific application parameters and therapeutic effects of taVNS,with the underlying mechanisms remaining to be eluci-dated.This review comprehensively examines the application of taVNS in post-stroke motor rehabilitation,encompassing both asyn-chronous and synchronous combination with upper limb training,as well as monotherapy with taVNS.Its mechanisms of action include anti-inflammatory and neuroprotective effects,enhancement of blood-brain barrier integrity,and modulation of cortical excit-ability and neuroplasticity.The review further analyzed the stimulation parameters,safety,and efficacy of taVNS.However,high-quality clinical studies are still insufficient.The majority of taVNS research has focused on limb function scores without exploring the underlying neurophysiological mechanisms.There is no consensus on taVNS stimulation parameters,including stimulation inten-sity,frequency,duration,and timing.The safety profile of taVNS applied to the right ear warrants further validation.Limited studies have reported lateralized effects when taVNS is applied to the left ear.Future large-sample and high-quality studies are required to establish optimal stimulation parameters and elucidate the mechanisms of action,thereby informing the development of the most effective taVNS treatment protocols for clinical practice.
