目的 观察通督益神针疗法联合机器人导向性训练对脑卒中后上肢功能障碍患者的影响.方法 选择2023年1月—2024年2月在滨州市中医医院康复科治疗的脑卒中后上肢功能障碍患者96例.按照随机数字表法分为针刺组、机器人组和联合组,每组32例.针刺组在基础药物治疗和常规康复训练基础上接受通督益神针疗法留针治疗,1次/d,5次/周,持续治疗4周.机器人组在基础药物治疗和常规康复训练基础上接受机器人导向性训练,1次/d,20 min/次,5次/周,持续治疗4周.联合组接受通督益神针疗法联合机器人导向性训练,通督益神针疗法同针刺组,机器人导向性训练同机器人组,持续治疗4周.分别于治疗前后采用Fugl-Meyer上肢运动功能评定量表(FMA-UE)和Wolf上肢运动功能评价量表(WMFT)评估上肢运动功能;采用积木试验(BBT)评估手部运动功能;采用改良Barthel指数(MBI)评估日常生活活动能力.结果 ① FMA-UE评分:与治疗前比较,3组治疗后FMA-UE评分均明显升高(P<0.05).与针刺组、机器人组比较,联合组治疗后FMA-UE评分均明显更高(P<0.05).② WMFT评分:与治疗前比较,3组治疗后WMFT评分均明显升高(P<0.05).与针刺组、机器人组比较,联合组治疗后WMFT评分均明显更高(P<0.05).③ MBI和BBT评分:与治疗前比较,3组治疗后MBI和BBT评分均明显升高(P<0.05).与针刺组、机器人组比较,联合组治疗后MBI和BBT评分均明显更高(P<0.05).结论 通督益神针疗法联合机器人导向性训练可有效提高脑卒中患者上肢运动功能、手部功能和日常生活活动能力.

Objective To evaluate the effects of Tongdu Yishen acupuncture therapy combined with robot-guided training on upper limb dysfunction in post-stroke patients.Methods A total of 96 patients with upper limb motor dysfunction subsequent to stroke treated in the Rehabilitation Department of Binzhou Traditional Chinese Medicine Hospital from January 2023 to February 2024 were enrolled.All participants were randomly divided into acupuncture group,robot group and combined group,with 32 cases in each group.The acupuncture group received Tongdu Yishen acupuncture therapy with needle retention in addition to the basic drug treatment and conventional rehabilitation training,once a day,five times a week,for a total of four weeks.The robot group received robot-guided training in addition to the basic drug treatment and conventional rehabilitation training,once a day,20 minutes a time,five times a week,for a total of four weeks.The combined group received Tongdu Yishen acupuncture therapy combined with robot-guided training,with the same acupuncture intervention as the acupuncture group and the same robot training as the robot group,for a total of four weeks.Before and after treatment,the Fugl-Meyer Assessment for Upper Extremity(FMA-UE)and the Wolf Motor Function Test(WMFT)were used to assess the the upper limb motor function.The Box and Block Test(BBT)was used to assess the hand motor function.The Modified Barthel Index(MBI)was used to assess activities of daily living.Results(1)FMA-UE score:compared with those before treatment,the FMA-UE scores of all three groups increased significantly after treatment(P<0.05).Compared with the acupuncture group and the robot group,the FMA-UE scores of the combined group were significantly higher after treatment(P<0.05).(2)WMFT score:compared with those before treatment,the WMFT scores of all three groups increased significantly after treatment(P<0.05).Compared with the acupuncture group and the robot group,the WMFT scores of the combined group were significantly higher after treatment(P<0.05).(3)MBI and BBT scores:compared with those before treatment,the MBI and BBT scores of all three groups increased significantly after treatment(P<0.05).Compared with the acupuncture group and the robot group,the MBI and BBT scores of the combined group were significantly higher after treatment(P<0.05).Conclu-sion The Tongdu Yishen acupuncture therapy combined with robot-guided training can effectively enhance the upper limb motor function,hand function and activities of daily living of stroke patients.
