慢性肾脏病患者血清Klotho蛋白与心血管疾病的相关性分析OA
Correlation analysis between serum Klotho protein and cardiovascular diseases in patients with chronic kidney disease
目的 探讨慢性肾脏病(CKD)患者血清克罗索蛋白(Klotho)与发生心血管疾病(CVD)的相关性.方法 基于2009-2016年美国国家健康与营养调查(NHANES)数据库的数据进行横断面分析,采用多元logistic回归、亚组分析,并用倾向性评分匹配减少混杂变量的影响.结果 纳入的2 017例患者中,合并CVD者504例,不合并CVD者1 513例.logistic回归分析结果显示,3个模型中均发现CKD患者Klotho水平升高与CVD发生率之间存在负相关(P<0.05).随着Klotho水平由低到高(Q1~Q4),CVD发生风险呈梯度式下降(P<0.05).在模型3中,以Q1 为参考,Q2~Q4 对应的OR分别为0.94(95%CI:0.70~1.26)、0.88(95%CI:0.65~1.19)和0.72(95%CI:0.52~0.99).亚组分析结果显示,在女性、>60岁、eGFR≥60 mL·min-1·(1.73 m2)-1、BMI<28 kg/m2 的患者中,Klotho水平对发生CVD的保护作用更明显(P<0.05).采用倾向性评分匹配并调整混杂因素后,Klotho仍是发生CVD的独立保护因素(P=0.026,OR=0.631,95%CI:0.417~0.942).结论 血清 Klotho是CKD患者发生CVD的独立保护因素,在特定亚组中效果更明显.
Objective To investigate the correlation between serum Klotho protein and the occurrence of cardiovascular disease(CVD)in patients with chronic kidney disease(CKD).Methods We conducted a cross-sectional analysis using data from the National Health and Nutrition Examination Survey(NHANES)from 2009 to 2016.Multivariate logistic regression,subgroup analysis,and propensity score matching were a-dopted to reduce the impact of confounding variables.Results A total of 2 017 patients were enrolled,among whom 504 had comorbid CVD and 1 513 did not.Logistic regression analysis showed that in all three models,elevated Klotho levels were negatively correlated with the incidence of CVD in CKD patients(P<0.05).As Klotho levels increased from the lowest to the highest quartile(Q1 to Q4),the risk of CVD decreased in a gra-dient manner(P<0.05).In Model 3,with Q1 as the reference,the odds ratios(OR)for Q2,Q3,and Q4 were 0.94(95%CI:0.70 to 1.26),0.88(95%CI:0.65 to 1.19),and 0.72(95%CI:0.52 to 0.99),respectively.Subgroup analysis demonstrated that the protective effect of Klotho levels against CVD was more significant in female patients,those aged over 60 years,those with estimated glomerular filtration rate(eGFR)≥60 mL·min-1·(1.73 m2)-1,and those with body mass index(BMI)<28 kg/m2(P<0.05).After propensity score matching and adjustment for confounding factors,Klotho remained an independent protective factor for the occurrence of CVD(P=0.026,OR=0.631,95%CI:0.417-0.942).Conclusion Serum Klotho is an in-dependent protective factor against CVD in CKD patients,and its effect is more pronounced in specific sub-groups.
