骨髓增生异常性肿瘤向急性髓系白血病转化的影响因素及预后分析OA
Analysis of influencing factors and prognosis of the transformation from myelodysplastic syndromes to acute myeloid leukemia
目的 探讨骨髓增生异常性肿瘤(MDS)患者向急性髓系白血病(AML)转化的影响因素,并评估其对患者预后的影响.方法 回顾性分析2015年4月至2023年6月在贵州医科大学附属医院确诊为 MDS的281例患者临床资料,根据随访期间是否转化为AML将患者分为转白组(n=84)和非转白组(n=197).收集两组实验室数据、基因突变及染色体异常结果,通过逐步logistic回归分析 MDS向 AML转化的危险因素,并采用Kaplan-Meier法进行生存分析,Cox回归模型评估预后的影响因素.结果 与非转白组比较,转白组年龄[(64.05±14.90)岁 vs.(59.32±15.89)岁]更大,骨髓纤维化程度、基因突变率(57.1%vs.31.0%)、染色体异常率(54.8%vs.27.4%)、修订版国际预后评分系统(IPSS-R)风险等级高危(31.0%vs.19.3%)和极高危占比(27.3%vs.7.6%)、IPSS-R评分[(3.87±1.52)分 vs.(2.91±1.43)分]更高,差异有统计学意义(P<0.05).多因素logistic回归分析结果显示,基因突变、RUNX1突变、染色体异常、8q三体、IPSS-R等级及评分是 MDS向AML转化的独立影响因素(P<0.05).转白组中位总生存(OS)时间明显短于非转白组(17.58个月 vs.58.31个月,P<0.05).Cox回归模型分析结果显示,年龄、转白状态、基因突变、RUNX1突变、8q三体、IPSS-R等级极高危、IPSS-R评分是 OS时间的独立影响因素(P<0.05).结论 MDS向 AML转化预示极差的生存结局,而RUNX1突变、8q三体及IPSS-R评分是转化与预后的共同独立影响因素.
Objective To investigate the influencing factors for leukemic transformation to acute mye-loid leukemia(AML)in patients with myelodysplastic syndromes(MDS),and to evaluate their impact on the prognosis.Methods A retrospective analysis was conducted on the clinical data of 281 patients diagnosed with MDS at The Affiliated Hospital of Guizhou Medical University from April 2015 to June 2023.The patients were divided into the transformation to AML group(n=84)and the non-transformation to AML group(n=197)based on whether they developed AML during the follow-up period.Laboratory data,gene mutations,and chromosomal abnormalities results of both groups were collected.Stepwise logistic regression analysis was used to identify the risk factors for MDS transformation to AML,and the Kaplan-Meier method was employed for survival analysis.The Cox regression model was used to evaluate the influencing factors of prognosis.Re-sults Compared with the non-transformation group,the age of the transformation group was larger[(64.05±14.90)years vs.(59.32±15.89)years],the degree of bone marrow fibrosis,the rate of gene muta-tions(57.1%vs.31.0%),the rate of chromosomal abnormalities(54.8%vs.27.4%),the high-risk(31.0%vs.19.3%)and extremely high-risk(27.3%vs.7.6%)proportions of international prognostic sco-ring system-revised(IPSS-R)risk grades,and the IPSS-R score(3.87±1.52 vs.2.91±1.43)were higher,the differences were statistically significant(P<0.05).The results of multivariate logistic regression analysis showed that gene mutations,RUNX1 mutations,chromosomal abnormalities,trisomy 8,IPSS-R grade and score were independent influencing factors for MDS transformation to AML(P<0.05).The median overall survival(OS)time of the transformation group was significantly shorter than that of the non-transformation group(17.58 months vs.58.31 months,P<0.05).The results of Cox regression model analysis showed that age,transformation status,gene mutations,RUNX1 mutations,trisomy 8,extremely high-risk IPSS-R grade,and IPSS-R score were independent influencing factors for OS time(P<0.05).Conclusion The transforma-tion of MDS to AML indicates a very poor survival outcome,while RUNX1 mutations,trisomy 8 and IPSS-R score are common independent influencing factors for both the transformation and prognosis.
