Neurodermatitis is characterized by lichenoid skin lesions and paroxysmal intense itching,with a prolonged course and high recurrence rate.Western medical treatments,particularly corticosteroids,face challenges such as unstable efficacy,high relapse rates,and significant side effects.Based on Guo Ziguang's"Human-Symptom-Disease-Syndrome"system,this study constructs a novel TCM diagnostic and therapeutic approach.Breaking through the limitations of traditional syndrome differentiation,it proposes for the first time that"Wind-Damp-Heat-Toxin stasis obstructing the skin,with Lung-Spleen deficiency"serves as the common core pathogenesis.A hierarchical framework of"human(holistic)-symptom(main manifestations)-disease(shared pathogenesis)-syndrome(individual variations)"is established to clarify recurrence mechanisms.Dynamic stage-specific treatment is implemented:in the acute phase,in-sect-based medications combined with Heat-clearing and Blood-cooling herbs are emphasized to rapidly interrupt the itch-scratch cycle,while the remission phase shifts to Lung-Spleen tonification,Blood activation,and collateral dredging to consolidate therapeutic effects and prevent relapse.A foundational formula,"Zhiyang Yufu Decoction",was developed,with modular modifications based on syn-drome types,forming a"one-core-formula,multi-syndrome adaptation"model.This protocol integrates hierarchical differentiation with dynamic stage adjustment,balancing rapid itch relief,standardized operation,and long-term recurrence prevention.It offers a new paradigm for neurodermatitis treatment that harmonizes TCM holistic principles with modern diagnostic logic.
