This article explores clinical experience in treating trigeminal neuralgia(TN)from the perspective of"stagnated Heat in Yang-ming transforming into Toxicity and generating Wind".It is proposed that TN predominantly affects the areas distributed by the Yangming Meridian.The core pathogenesis involves pathogenic factors stagnating in Yangming,leading to Qi blockage and congestion.This stagnated Heat transforms into Fire and accumulates Toxicity.The Fire-Toxin travels along the meridians and enters the collaterals,generating Wind and stirring Blood,resulting in Qi and Blood disorder manifesting as pain and convulsions.In chronic cases,the pathogen becomes deeply entrenched in the collaterals,making the disease intractable and prone to relapse.Treatment should focus on clearing and dispersing the stag-nated Heat in Yangming and purging Fire to resolve Toxicity as the fundamental approach,with staged treatment strategies.During the acute phase,emphasis is placed on dispelling pathogens and relieving stagnation,combined with methods like"taking drastic measures to solve a problem at source"and cooling Blood to resolve Toxicity.For prolonged illness,agents that search out phlegm,dispel stasis,unblock collat-erals,and alleviate pain should be added to prevent recurrence.The formula"Xijiao Shengma Decoction"directly addresses the pathogenesis of"stagnated Heat in Yangming transforming into Toxicity".It can dispel Wind,clear Heat,cool Blood,and resolve Toxicity,serving as a representative formula for treating TN.This provides a new perspective for the TCM treatment of trigeminal neuralgia.
