骨科洗药熏洗治疗膝关节滑膜炎痰饮流注证的临床疗效及安全性观察OA
Clinical efficacy and safety of Guke Xiyao Fumigation treatment for patients with phlegm-fluid abscess type knee synovitis
目的 观察骨科洗药熏洗治疗膝关节滑膜炎痰饮流注证的临床疗效与安全性.方法 选择2022年12月—2023年12月首都医科大学附属北京中医医院骨伤科门诊就诊的膝关节滑膜炎痰饮流注证患者72例,按随机数字表法分为观察组、对照组,各36例.观察组采用骨科洗药熏洗,每天1次,连续治疗5 d,休息2 d,总计治疗4周.对照组予代温灸膏外敷,连续贴敷5 d,休息2 d,总计治疗4周.对比2组治疗前后疼痛数字评价量表(numerical rating scale,NRS)评分、西安大略和麦克马斯特大学骨关节炎指数(Western Ontario and McMaster-Universities Osteoarthritis Index,WOMAC)评分、浮髌试验分级、膝关节周径、安全性.结果 治疗后,2组NRS评分均低于治疗前(P<0.05);观察组治疗前后改善差值高于对照组(P<0.05).治疗后,2组WOMAC评分均低于治疗前(P<0.05);观察组治疗前后改善差值高于对照组(P<0.05).治疗后,2组膝关节周径均较治疗前减少,但差异无统计学意义(P>0.05);但观察组治疗前后改善差值高于对照组(P<0.05).治疗后,观察组浮髌试验Ⅰ级占比、浮髌试验改善情况高于对照组,但差异无统计学意义(P>0.05).观察组的不良反应总发生率为8.33%,低于对照组的30.56%(P<0.05).结论 骨科洗药熏洗治疗膝关节滑膜炎痰饮流注证能有效缓解疼痛、改善膝关节功能与肿胀,且安全性良好.
Objective To observe the clinical efficacy and safety of Guke Xiyao Fumigation in the treatment of knee synovitis with phlegm-fluid abscess syndrome.Methods A total of 72 patients with knee synovitis of phlegm-fluid abscess type,admitted to the Department of Orthopedics,Beijing Hospital of Traditional Chinese Medicine,Capital Medical University,from December 2022 to December 2023,were enrolled and randomly divided into an observation group and a control group using a random number table,with 36 cases in each group.The observation group received Guke Xiyao Fumigation once daily for 5 consecutive days followed by a 2-day rest,repeated for a total of 4 weeks.The control group received topical application of Daiwen Moxibustion Plaster,applied for 5 consecutive days followed by a 2-day rest,for a total treatment duration of 4 weeks.Clinical outcomes were compared between the two groups before and after treatment,including the Numerical Rating Scale(NRS)score,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC),floating patella test grading,knee circumference,and safety evaluation.Results After treatment,NRS scores in both groups were significantly lower than those before treatment(P<0.05).The improvement difference between pre-and post-treatment was greater in the observation group than in the control group(P<0.05).WOMAC scores in both groups were also significantly reduced after treatment(P<0.05),with greater improvement in the observation group(P<0.05).Knee circumference was reduced in both groups after treatment;however,the between-group difference was not statistically significant(P>0.05),while the within-group improvement difference was greater in the observation group than in the control group(P<0.05).After treatment,the proportion of grade I floating patella test and overall improvement in the observation group were higher than those in the control group,but the difference was not statistically significant(P>0.05).The total incidence of adverse reactions in the observation group was 8.33%,which was lower than 30.56%in the control group(P<0.05).Conclusion Guke Xiyao Fumigation is effective in relieving pain,improving knee joint function,and reducing swelling in patients with knee synovitis of phlegm-fluid abscess syndrome,with good safety.
