基于数据挖掘的针刺治疗运动性疲劳选穴规律分析OA
Analysis of Acupoint Selection Rules for Acupuncture Treatment of Exercise-induced Fatigue Based on Data Mining
目的:针对运动性疲劳(EIF)发病率高、易累积致机体损伤的公共卫生问题,运用数据挖掘技术分析针刺治疗EIF的临床随机对照试验(RCT)文献,系统总结其选穴、归经及配伍规律,为优化抗疲劳针灸方案提供循证依据.方法:检索中国知网、维普、万方三大数据库,时限为建库至2025年3月1日,以"运动性疲劳"AND"针灸/针刺"为主题词组合检索.采用Excel 2019建立针灸处方数据库,录入腧穴频次、部位、归经等参数;运用SPSS Modeler 14的Apriori算法(支持度≥10%、置信度≥80%)挖掘关联规则,SPSS Statistics 20进行高频腧穴聚类分析.结果:共纳入合格RCT文献38篇,涉及38个腧穴(含复合穴组),总使用频次141次.高频穴位(频次≥3)共18个,占比47.37%,累计频次78.72%;前八位高频穴(足三里、关元、三阴交、气海、合谷、阳陵泉、肾俞、百会)占比57.45%,其中足三里以21.28%的绝对优势居首.归经分布以足阳明胃经最多(26.24%),其次为任脉(17.02%),覆盖除手少阳三焦经外的13条正经及任督二脉.取穴部位以下肢部为核心(56.74%),胸腹部次之(17.02%).特定穴中五输穴占比最高(34.04%),下合穴次之(25.53%).关联规则显示,关元→足三里的支持度最高(39.74%),气海+足三里→关元的增益达2.53倍.聚类分析将18个高频穴分为5类,足三里、关元为早期聚类核心.结论:针刺治疗EIF以"多气多血"的足阳明胃经为主,辅以"阴脉之海"任脉,核心穴位为足三里、关元、三阴交、气海,体现了"培补后天、调和气血"的治则.当前研究仍存在高质量证据不足、诊疗标准不统一等问题,未来需开展多中心大样本RCT,构建循证针灸处方体系.
Objective:To address the public health issue of high incidence and cumulative tissue damage caused by exercise-induced fatigue(EIF),this study applied data mining techniques to analyze the randomized controlled trial(RCT)literature on acupuncture treatment for EIF,aiming to systematically summarize acupoint selection rules,meridian affiliations,and compatibility patterns,and provide evidence-based references for optimizing anti-fatigue acupuncture protocols.Methods:Three major Chinese databases(CNKI,VIP,and Wanfang)were searched from their inception to March 1,2025,using the search strategy of"exercise-induced fatigue"AND"acupuncture/moxibustion".An acupuncture prescription database was established using Excel 2019 to record acupoint frequency,body region,meridian affiliation,etc.Association rule mining was performed via the Apriori algorithm in SPSS Modeler 14(support≥10%,confidence≥80%),and cluster analysis of the high-frequency acupoints was conducted using SPSS Statistics 20.Results:A total of 38 eligible RCTs were included,involving 38 acupoints(including compound acupoint groups)with 141 total applications.There were 18 high-frequency acupoints(frequency≥3),accounting for 47.37%of total acupoints and 78.72%of total applications.The top eight high-frequency acupoints(Zusanli ST36,Guanyuan CV4,Sanyinjiao SP6,Qihai CV6,Hegu LI4,Yanglingquan GB34,Shenshu BL23,and Baihui GV20)accounted for 57.45%of total applications,with Zusanli ranking first(21.28%).In terms of meridian distribution,the Foot-Yangming Stomach Meridian was the most frequently used(26.24%),followed by the Ren Meridian(17.02%),covering 13 regular meridians(except the Hand-Shaoyang Sanjiao Meridian)and the Ren/Du meridians.Acupoint selection focused on the lower limbs(56.74%)as the core region,followed by the chest and abdomen(17.02%).Among specific acupoints,the Five-Shu points accounted for the highest proportion(34.04%),followed by Lower-He points(25.53%).Association rule analysis showed that the support for"Guanyuan→Zusanli"was the highest(39.74%),and the lift for"Qihai+Zusanli→Guanyuan"reached 2.53.Cluster analysis classified the 18 high-frequency acupoints into 5 categories,with Zusanli and Guanyuan as the early clustering cores.Conclusion:Acupuncture treatment for EIF primarily targets the Foot-Yangming Stomach Meridian("abundant in qi and blood")supplemented by the Ren Meridian("sea of yin"),with core acupoints including Zusanli,Guanyuan,Sanyinjiao,and Qihai,reflecting the principle of"cultivating the acquired constitution and harmonizing qi and blood".Current limitations include insufficient high-quality evidence and inconsistent diagnostic/treatment standards.Future studies should conduct multicenter large-sample RCTs to establish evidence-based acupuncture prescription systems.
