目的:基于静息态功能磁共振成像(rs-fMRI)研究推拿治疗腰椎间盘突出症的潜在中枢机制.方法:将72例腰椎间盘突出症患者随机分为对照组(36例)和治疗组(36例,脱落1例),对照组采用经皮神经电刺激配合牵引治疗,治疗组则采用推拿治疗,每天治疗1次,两组均进行14次治疗.比较两组患者治疗前后疼痛视觉模拟评分(VAS评分)和抑郁自评量表评分(SDS评分);治疗结束后采集rs-fMRI数据,观察两组患者治疗后低频振幅比率(fALFF)的变化.结果:治疗前,两组患者VAS及SDS评分组间差异均无统计学意义,具有可比性(P>0.05).治疗后,两组患者VAS评分及SDS评分均较治疗前显著降低,差异有统计学意义(P<0.05);治疗组VAS评分与对照组相比,差异无统计学意义(P>0.05),但SDS评分显著低于对照组(P<0.05).治疗后,治疗组较对照组fALFF值在左角回(Angular.L)降低,左侧额下回三角部(IFGtri.L)升高,差异有统计学意义(P<0.05).结论:推拿可能通过调节腰椎间盘突出症患者左角回及左侧三角部额下回的局部脑功能,缓解疼痛,改善抑郁情绪.

Objective:To study the potential central mechanism of Tuina in the treatment of lumbar disc herniation based on resting-state functional magnetic resonance imaging(rs-fMRI).Methods:A total of 72 patients with lumbar disc herniation were randomly divided into the control group(36 cases)and the treatment group(36 cases,1 case dropped out).The control group was treated with transcutaneous electrical nerve stimulation combined with traction,while the treatment group was treated with Tuina,once a day.Both groups were treated for 14 times.The visual analogue scale(VAS score)and self-rating depression scale(SDS score)were compared between the two groups before and after treatment.After treatment,rs-fMRI data were collected to observe the changes of low frequency amplitude ratio(fALFF)in the two groups.Results:Before treatment,there was no significant difference in VAS and SDS scores between the two groups(P>0.05).After treatment,the VAS score and SDS score of the two groups were significantly lower than those before treatment,and the difference was statistically significant(P<0.05).There was no significant difference in VAS score between the treatment group and the control group(P>0.05),but the SDS score was significantly lower than that of the control group(P<0.05).After treatment,the fALFF value of the treatment group was lower than that of the control group in the left angular gyrus(Angular.L),and the left inferior frontal gyrus triangle(IFGtri.L)was increased,and the difference was statistically significant(P<0.05).Conclusion:Tuina may relieve pain and improve depression by regulating the local brain function of the left angular gyrus and left triangular part of inferior frontal gyrus in patients with lumbar disc herniation.
