Background:Gan-Du-Qing(GDQ),a traditional herbal formula containing Astragali radix(Huangqi)and Belamcandae rhizoma(Shegan),is used for respiratory disorders,but evidence for protecting against PM2.5-related lung impairment is limited.This study evaluated GDQ on pulmonary function and quality of life in individuals exposed to PM2.5 and explored potential mechanisms.Methods:In a randomized,double-blind,placebo-controlled trial,sanitation workers(high exposure)and indoor staff from an Environmental Protection Bureau(low exposure)received GDQ or placebo for 4 weeks and were followed for 12 weeks with two follow-up visits.Outcomes included vital signs,pulmonary function,SF-36,COOP/WONCA,and gut microbiota profiling by 16S rRNA.Results:In the high-exposure group,GDQ improved pulmonary function,SF-36 and COOP/WONCA scores,and mitigated gut microbiota dysbiosis.PM2.5 exposure was associated with restrictive ventilatory changes.While between-group differences were not significant immediately post-intervention,FEV1,FEF50%,and FEF75%improved at the second follow-up,suggesting a delayed benefit.High exposure increased gut microbial diversity and abundance,which were partially reduced after GDQ(P<0.05).Conclusion:GDQ may alleviate PM2.5-related chronic pulmonary injury,potentially via anti-inflammatory and immunomodulatory effects mediated by gut microbiota modulation.
