基于真实世界的电针治疗慢性萎缩性胃炎临床疗效分析OA
Analysis of Clinical Efficacy of Electroacupuncture in the Treatment of Chronic Atrophic Gastritis Based on Real World
目的 评价电针治疗慢性萎缩性胃炎(CAG)的临床综合疗效.方法 采用前瞻性、自身前后对照研究设计,选择2023年9月—2024年5月于安徽中医药大学第一附属医院针灸康复科门诊就诊的30例慢性萎缩性胃炎患者,均接受电针治疗,每周治疗2次,10次为1个疗程,疗程间隔1周,共治疗20周.观察治疗前后患者中医症状积分和总积分、胃镜病理组织学(萎缩、肠化、异型增生、炎性活动)积分和总积分、HAMA、HAMD评分、血清胃功能指标(PGⅠ、PGR、G-17、PGⅡ)变化情况,以及中医证候疗效和胃镜疗效.结果 中医证候总有效率93.33%(28/30),胃镜总有效率83.33%(25/30).与治疗前比较,治疗后患者各项中医症状积分和总积分均明显下降(P<0.05);萎缩、肠化及炎性活动病理组织学积分和总积分HAMA、HAMD评分均显著降低(P<0.01),但异型增生的病理组织学积分变化不明显,差异无统计学意义(P>0.05);血清PGⅠ、PGR、G-17指标较治疗前升高(P<0.05),PGⅡ较治疗前降低(P<0.05).结论 电针疗法可有效缓解CAG患者的临床症状,显著改善胃镜及组织病理学表现,减轻焦虑抑郁状态,调节血清胃功能指标,具有较好的临床疗效和安全性.
Objective To evaluate the clinical efficacy of electroacupuncture in the treatment of chronic atrophic gastritis(CAG).Methods A prospective,self-controlled study was conducted.A total of 30 CAG patients in the Outpatient Department of Acupuncture and Rehabilitation in The First Affiliated Hospital of Anhui University of Chinese Medicine from September 2023 to May 2024 were enrolled.All patients received electroacupuncture treatment.The treatment schedule was set at two sessions per week.Ten sessions constituted one treatment course,followed by a one-week interval.The entire intervention comprised two courses,for a total treatment duration of 20 weeks.Changes in traditional Chinese medicine(TCM)symptom individual score and total scores,gastroscopic histopathology(atrophy,intestinal metaplasia,dysplasia,and inflammatory activity)individual score and total scores,HAMA(Hamilton Anxiety Rating Scale)scores,HAMD(Hamilton Depression Rating Scale)scores,serum gastric function indicators(PGⅠ,PGR,G-17,and PGⅡ)were observed before and after treatment.The efficacy of TCM syndromes and gastroscopic efficacy were assessed.Results The total effective rate of TCM syndromes was 93.33%(28/30),while the total effective rate of gastroscopy was 83.33%(25/30).Compared with treatment before,All TCM symptom individual and total scores decreased significantly(P<0.05);Histological individual and total scores of atrophy,intestinal metaplasia,and inflammatory activity,as well as HAMA and HAMD scores,decreased significantly(P<0.01).While the histopathological score for dysplasia showed no significant change,and the difference was not statistically significant.(P>0.05);Serum indicators including PGⅠ,PGR,and G-17 increased(P<0.05),while PGⅡ decreased(P<0.05).Conclusion Electroacupuncture can effectively relieve clinical symptoms of CAG patients,significantly improve gastroscopic and histopathology findings,alleviate anxiety and depression,and regulate serum gastric function indicators.It exhibits favorable clinical efficacy and a good safety profile.
