目的:基于卵巢颗粒细胞凋亡探究温经汤改善雷公藤所致卵巢储备功能下降(decreased ovarian re-serve,DOR)的机制.方法:构建 DOR 大鼠模型,分为模型对照组、芬吗通组(序贯疗法:第 1~2 d 灌胃雌二醇片0.2 mg/kg,第3~5 d 灌胃雌二醇地屈孕酮片1.2 mg/kg)、温经汤4.85、9.7、19.4 g/kg 组.给药4 周后,观察大鼠动情周期并测量卵巢面积;HE 染色观察卵巢结构并计数卵泡数;TUNEL 法检测卵巢颗粒细胞凋亡;Western blot法检测卵巢 Bcl-2 相关 X 蛋白(BAX)、B 细胞淋巴瘤/白血病-2(BCL-2)蛋白表达.使用 GTW 刺激人卵巢颗粒细胞瘤细胞(KGN),CCK-8 法检测温经汤血清对 KGN 细胞增殖的影响,将细胞设为空白对照组、模型对照组、5%正常血清组、5%温经汤含药血清组.流式细胞术检测细胞凋亡情况;Western blot 检测细胞 BAX、BCL-2 蛋白表达.结果:与正常对照组相比,模型对照组大鼠动情周期紊乱率显著升高,卵巢面积显著减小,颗粒细胞凋亡率显著增加(P<0.01),卵泡结构受损、颗粒细胞排列散乱,原始卵泡、初级卵泡、次级卵泡、窦卵泡数显著减少,闭锁卵泡数量明显增加(P<0.05 或 P<0.01),卵巢组织 BAX 蛋白表达显著上调,BCL-2 蛋白表达显著下调(P<0.01);与模型对照组相比,各给药组卵泡结构损伤改善、卵巢颗粒细胞排列整齐,芬吗通组及温经汤9.7、19.4 g/kg 组动情周期紊乱率显著降低,卵巢面积显著增大,卵巢颗粒细胞凋亡率明显减少(P<0.05 或 P<0.01),闭锁卵泡数明显减少(P<0.05),原始卵泡、窦卵泡数明显增加(P<0.01 或 P<0.05),温经汤 9.7 g/kg、芬吗通组初级卵泡数明显增加(P<0.01 或 P<0.05),各给药组 BAX 蛋白表达明显下调(P<0.01 或 P<0.05),温经汤 19.4 g/kg 组 BCL-2 蛋白表达明显上调(P<0.05).与空白对照组相比,模型对照组细胞存活率显著降低,早期凋亡率和总细胞凋亡率显著升高(P<0.01),BAX 蛋白表达明显下调,BCL-2 蛋白表达上调(P<0.05);与模型对照组相比,5%空白血清组上述指标无明显变化,9.7 g/kg 温经汤5%含药血清组细胞存活率显著升高,早期凋亡率和总凋亡率明显降低(P<0.05 或 P<0.01),BAX 蛋白表达明显下调,BCL-2 蛋白表达明显上调(P<0.05).结论:温经汤通过减少颗粒细胞凋亡,维持卵泡结构完整性及减少闭锁卵泡数量,进而改善 DOR 大鼠卵巢储备功能.

Objective:To explore the mechanism of Wenjing(温经)Decoction in ameliorating Leigongteng(雷公藤)-induced diminished ovarian reserve(DOR)based on ovarian granulosa cell apoptosis.Methods:Administration of Leigongteng glycosides(GTW)was used to establish a rat model of DOR by observing the estrous cycle.Then,the rats were assigned into the model control,Femoston(sequential therapy:gavage of 0.2 mg/kg estradiol tablets on days 1-2,and gavage of 1.2 mg/kg estradiol and dydrogesterone tablets on days 3-5),and Wenjing Decoction(4.85,9.7,and 19.4 g/kg)groups.After 4 weeks of treatment,the estrous cycle was observed and the ovarian area was measured.HE staining was employed for ovarian structure observation and follicle counting.Ovarian granulosa cell apoptosis was detec-ted by the terminal deoxynucleotidyl transferase dUTP nick-end labeling assay.The protein levels of B-cell lymphoma/leukemia-2(BCL-2)and BCL-2-associated X protein(BAX)in the ovarian tissue were measured by Western blot.The human granulosa-like tumor cell line(KGN)was cultured in vitro and stimulated with GTW.The proliferation of KGN cells was detected by the CCK8 assay.The cells were allocated into normal control,model control,5%blank serum,and 5%Wenjing Decoction-containing serum groups.The apoptosis was detected by flow cytometry,and the protein levels of BAX and BCL-2 were measured by Western blot.Results:In the animal experiment,compared with the blank control group,the model control group presented increased estrous cycle disorder rate,reduced ovarian area,and increased ovari-an granulosa cell apoptosis rate(P<0.01),as well as disrupted follicular structure with granulosa cell disarrangement,reduced number of primordial follicles,primary follicles,secondary follicles,and antral follicles,and increased number of atretic follicles(P<0.01 or P<0.05),up-regulated protein level of BAX,and down-regulated protein level of BCL-2(P<0.01).Compared with the model control group,drug treatment groups showed improved follicular structure and granulosa cell arrangement,and the Femoston and Wenjing Decoction(9.7 g/kg and 19.4 g/kg)groups demonstrated reduced estrous cycle disorder rate,increased ovarian area,and decreased ovarian granulosa cell apoptosis rate(P<0.05 or P<0.01),and reduced number of atretic follicles(P<0.05),and increased number of primordial and antral follicles(P<0.01 or P<0.05).Wenjing Decoction(9.7 g/kg)and Femoston groups showed increased number of primary folli-cles(P<0.01 or P<0.05).The BAX protein level was down-regulated in all treatment groups(P<0.01 or P<0.05),and the BCL-2 protein level was up-regulated in the Wenjing Decoction(19.4 g/kg)group(P<0.05).In the cell ex-periment,compared with the blank control group,the model control group showed reduced cell viability,increased early and overall apoptosis rates(P<0.01),down-regulated BAX expression,and up-regulated BCL-2 expression(P<0.05).Compared with the model control group,no significant changes in the aforementioned parameters were observed in the 5%blank serum group,while the 5%Wenjing Decoction-containing serum group presented increased cell viability,reduced early and overall apoptosis rates(P<0.01 or P<0.05),down-regulated BAX expression,and up-regulated BCL-2 expres-sion(P<0.05).Conclusion:Wenjing Decoction inhibits granulosa cell apoptosis,maintains follicular structural integri-ty,and reduces the number of atretic follicles,thereby restoring ovarian reserve function in DOR rats.
